Common Complications in Lasik Surgery- Peri -Operative Early Flap Dislocation and Epithelial Ingrowth

LASIK surgery has become immensely popular as a safe and quick form of Refractive Corrective Surgery. One of the reasons for the lasik eye surgery in NJ to be immensely popular is its low incidence of complications. However it must be remembered that it is an operative procedure and even if it is accomplished by the best lasik doctor, some risk percentage is always there.   One such possibility in lasik procedure is peri -operative early flap dislocation which can happen in less than a week after the LASIK surgery. 1 to 2 percent incidence has been reported. The main cause of this situation is due to mechanical displacement by lid action particularly if the ocular surface is dry in the early postoperative period. The late onset of the traumatic LASIK flap dislocation in varying periods from a week to 7 years after the LASIK procedure has intensified the question about the adhesive strength between the flap and the stromal bed.

Any kind of traumatic late flap dislocation post lasic eye surgery must be treated as an ocular emergency. Patients with Traumatic late flap dislocations have higher risk for flap folds, epithelial ingrowth , DLK and infection. In such a condition, the usual procedure is to raise the flap carefully and the interface is carefully examined for any epithelial cells or other debris. The flap and the corneal bed should be adequately scraped before the flap is repositioned. For additional protection, a contact lens can be applied to provide extra protection from further displacement and to protect the flap from eyelid movement. The patient should be adequately informed about such complications before taking consent for carrying out the lasik eye surgery. Patient who are involved in aggressive contact sports or in jobs which require working in dusty environment must be cautioned to take adequate protection to preempt any eye injury after lasik eye surgery new jersey.

Another common complication which is seen after lasik treatment is post-LASIK epithelial ingrowth . Epithelial ingrowth (EI) is a post Lasik migration of epithelial cells from the edges of the flap into the interface. The incidence of EI has considerably reduced in the last few years due to due to sophistication in surgical techniques and perfection in microkeratome technology and with the introduction of femtosecond laser flap creation. Most incidences of EI are trivial and do not require any corrective process. Persons with minimal visual loss and stable EI warrant only observation. However about 1% of patients can have complications which can result by blockages of aqueous diffusion and flap nutrition and/or by producing proteolytic enzymes, irregular astigmatism, foreign body sensation, and visual impairment by extension to the visual axis. If the EI progresses towards the visual axis or about 2mm beyond the flap edge with significant loss of vision could require surgical intervention.
