Nervous system consist of :- 1- Central nervous system * CNS* which includes :- # Brain and spinal cord 2- Peripheral nervous system* PNS* which includes:- # 12 pairs of cranial nerves # 32 pairs of spinal nerves @ nursetobe __

WHAT IS SPINAL INJURY ? Spinal cord is a part of the central nervous system . It contain the nerves that carry messages between the brain and body . Having a spinal cord injury means presence of a very serious condition that can cause loss of movement(paralysis) below the injured site. @ nursetobe __

Spinal cord consist of 31 different segments :- 7 cervical segments (c1-c2-c3-c4-c5-c6-c7) 12thoracic segments (TH1-TH2-TH3-TH4-TH5-TH6-TH7....) 5 Lumber segments (L1-L2-L3....) 5 sacral segments 1 coccygeal segments @ nursetobe __

Cervical injury :- If the injury is in the high - cervical (C1-C4) it consider as the most sever injury . It causes paralysis in arms, legs and hands. It causes respiratory failure . Loss control of bowel and bladder . Reduce the ability of talking Require assistance with activities of daily living . While the injury in the low cervical ( c5-c7) is causing :- Corresponding nerves control arms and hands Those patient with this injury might be able to breath by their own and talk normaly . @ nursetobe __

Thoracic injury :- It causes weakness of the mid- back and abdominal muscles Arms and hands usually moving normally . Legs are losing their ability to move ( paraplegia ) Normal upper – body movement . They can use manual wheelchair @ nursetobe __

Lumbar and sacral injury :- Loss of function in the hip and legs . no voluntary control of bowel or bladder Depending on strength in the legs, may need a wheelchair and may also walk with braces @ nursetobe __

Causes of spinal cord injury :- RTA ( Road traffic accident ) . Fall from height . Trauma to the face/head/neck . Landing on the head during a sports injury . @ nursetobe __

Signs and symptoms of Spinal cord injury :- Numbness or tingling Weakness Difficulty in movement Paralysis Loss of bladder and bowel function . Unconsciousness Headache @ nursetobe __

Upper motor neuron damage in case of upper neuron damage it will result in :- Loss of control of movement . – Increase in the muscles tone The muscle size is unchanged but it is diminished slowly . Reflexes is increased * this type of paralysis known as spastic * Lower motor neuron damage In case of lower neuron damage it will result in :- The muscle continue to work but without control @ nursetobe __

Spinal shock It is a condition comes after the spinal injury . Characterized by losing the reflexes below the level of injury . “ The mechanism for spinal shock involves the sudden loss of conduction in the spinal cord as a result of the migration of potassium ions from the intracellular to extracellular spaces. This is associated with a transient loss of somatic and automatic reflex activity below the level of spinal cord segment damage ” @ nursetobe __

Some Spinal cord injury management : If you received a patient with known or potential spinal cord injury avoid moving the patient . Use neck collar to prevent the movement of the neck . Move patient using slide sheets . Use log roll with adequate personnel to turn the patient indwelling urinary catheter will be used @ nursetobe __

Nursing care plan of a patient with spinal core injury Will includes :- Risk for Ineffective Breathing Pattern Trauma, risk for [additional spinal injury] Impaired Physical Mobility Disturbed Sensory Perception Acute Pain Situational Low Self-Esteem Bowel Incontinence/Constipation Impaired Urinary Elimination Risk for Autonomic Dysreflexia Risk for Impaired Skin Integrity Knowledge, deficient ( nurseslab.ND ) @ nursetobe __
