1. Incomplete endorsement by floors : 

1. Incomplete endorsement by floors E.g. No Generic Names on the Medex 2 / Standing Order Sheet / Medication Record. Some medications are not yet started. Wrong or missing data on endorsed I & O. Disorganized patient’s chart.

2. Improper coordination and guidance when transferring pt. to neuro-icu : 

2. Improper coordination and guidance when transferring pt. to neuro-icu OR / ER / Ward Nurse is transferring patient at transition time of duty. OR / ER / Ward Nurse is late when transferring the patient. The patient sometimes comes first without the assistance of the Nurse.

3. Delayed encoding of patient’s account in the computer and delayed rendering of transfer slip from floors. :
