Benefits Enrollment Welcome to your benefits enrollment 2012. GES, like many employers, has experienced an increase in healthcare costs over the past year. Our challenge is to provide a good quality health plan for our employees and their families at a reasonable cost. GES with the help of our broker, USI, have worked together to find a solution to provide our employees with a new medical plan through our existing carrier, United Healthcare. There will be a few minor benefit changes. The Vision, Dental, Life and Disability benefits will not change. We are introducing an exciting new benefit, a medical expense and dependent care Flexible Spending Account, that will allow you to pay for your healthcare and dependent care expenses on a pre-tax basis. Something Different – a new enrollment process! This year we’ve made things easier for you! If you are currently enrolled in a medical plan option we will automatically enroll you into the new UHC Medical POS Plan with the same level of dependent coverage effective 3/1/12. The same goes for Vision and Dental. You only need to notify us if you wish to add, delete or change dependent coverage, or wish to waive your existing coverage. You will still need to enroll in the Short Term Disability, Supplement Life and new TASC Flexible Spending Account. Even if you wish to make no changes to your benefits you will still need to log onto the ADP Portal to verify your coverage.

Important Changes to Your Benefits Plans Medical - Effective 3/1/12 The existing UnitedHealthcare HMO and POS Plans will no longer be offered. They are being replaced by a new UnitedHealthcare POS Plus plan. In Network Plan changes include new Co-pays Physician: $15 co-pay for Primary Care/ $30 co-pay for Specialists Hospital: $0 co-pay for in-patient services, $150 co-pay for Emergency Room, $50 co-pay for Urgent Care Centers. Medical Insurance Overview UnitedHealthcare Choice Plus Traditional Plan With the POS Choice Plus Plan, you may choose to see any physician or healthcare professional, including specialists without a referral. You receive the highest level of benefits when you choose participating providers for services. However, you may receive care from non-participating providers and still receive benefits although you’ll be subject to a deductible, co-pay and out of pocket if you do so. In addition, the non-participating provider may balance bill you for costs that are not covered by UnitedHealthcare . Preventative services are covered at 100%, no deductible and no copayment for services such as; Annual physicals for adults and children, including well women GYN annual exams, mammograms and pap smears. Lab and x-rays provided in conjunction with preventative testing Scopic procedures such as routine colonoscopy and sigmoidoscopy and prostate cancer screening. Flexible Spending Accounts Rules: Governed by the IRS- only qualified eligible expenses can be reimbursed. A complete list of eligible and non-eligible expenses can be found in IRS Publication 502, www.irs.gov Employees must retain receipts for all purchases and provide them for verification if needed. FSA funds do not rollover – any unused funds left in your FSA accounts at the close of the Plan year are not refundable to you so choose your contribution level wisely. You will have a grace period of 30 days following the close of the plan year to incur and submit expenses towards any balance left in your FSA account and another 90 days to submit expenses manually for the past plan year. Any funds left in your account after that will be forfeited. Eligibility Eligibility Requirements You are eligible to participate in the Benefits Program if you are an active, full-time employee working at least 40 hours or more per week. Benefits will begin on the 1st of the month after 1 month of employment. Eligible Dependents include: · A spouse to whom you are legally married · A dependent child under age 26 · same sex and opposite sex Domestic Partners note :  documentation will be required in order to add an eligible dependent to your coverage. Life Event/Election Changes Your benefit elections will be binding for the Plan Year. Pre-tax Payroll Deductions Certain benefits described in this guide may be purchased with pre-tax payroll deductions as permitted by Section 125 of the Internal Revenue Code. When you purchase with pre-tax dollars, you reduce your taxable income, so fewer taxes are taken out of your paycheck. Remember Open Enrollment Begins 2/3 and Ends 2/09/12 Your verification of coverage , and any elections, changes or waivers must be entered into the ADP portal no later than the end of the business day , 2/09/12. Automatic Enrollment for Certain Benefits You will be automatically enrolled in the new UHC POS Plan with your existing dependent coverage. If you want to make changes to your current level of dependent coverage such as adding a new dependent or deleting coverage for an existing one or you want to waive coverage, you will need to make that change on the ADP Portal.
