In the Words of a Mother of a Pediatric Hospice Patient: “Hospice adds life to years, not just years to life.”

Objectives: : 

Objectives: Identify myths related to hospice; Identify diagnoses effectively managed with hospice; Identify components of hospice care; Identify opportunities for caring conversations and transition from aggressive to comfort care. It’s about how you LIVE.

Myth #1:Hospice requires a “DNR.” : 

Medicare regulations (Conditions of Participation for Hospice) do not require that a patient has completed a “Do Not Resuscitate” or other advance directive prior to or on admission to hospice. Many times a patient and family need the caring support of hospice to be able to make that decision. Myth #1:Hospice requires a “DNR.” It’s about how you LIVE.

Myth #3: Hospice means a person cannot return to the hospital. : 

Myth #3: Hospice means a person cannot return to the hospital. Through the Medicare Hospice benefit there is coverage for acute inpatient hospice hospitalization. A patient always has full control of their care plan and may choose to seek aggressive curative treatment at any given time. It’s about how you LIVE.

Myth #4: Once admitted to hospice all medications will be stopped. : 

Myth #4: Once admitted to hospice all medications will be stopped. Medications related to the terminal diagnosis will continue and many other medications will be added as needed to promote optimal comfort. It’s about how you LIVE.

Myth #5: Chemo, Dialysis, G-tube feedings are never covered by hospice. : 

Myth #5: Chemo, Dialysis, G-tube feedings are never covered by hospice. Medications are provided according to each patient’s unique needs. At times, chemotherapy, dialysis, tube feedings, and other more “aggressive” treatments are truly palliative. Depending on the rationale and goal of therapy these types of medications and treatments may be covered. It’s about how you LIVE.

Hospice Patients: : 

Hospice Patients: May or may not be bed-confined; May have a very high quality of life; May or may not have cancer; May or may not have pain; May or may not have an Advance Directive; May or may not have a 24 hour caregiver; May be any age – child, adult, elderly It’s about how you LIVE.

Who are hospice patients? : 

Who are hospice patients? Hospice patients are those patients for whom: no curative treatment exists; treatment options have been exhausted; or, the negative effects of treatment outweigh the medical benefit. It’s about how you LIVE.

Additional Hospice Diagnoses : 

Additional Hospice Diagnoses While these diagnoses give you a frame of reference, they are by no means all-inclusive. Each patient is an individual and should be evaluated as such. A referral for hospice screening and evaluation is appropriate for patients who: Have a life-limiting or terminal illness; Have a chronic illness that is not responding to treatment; And/Or: have determined they want no further aggressive or curative treatment. How is Hospice Paid For? :

Hospice Services are provided at: : 

Hospice Services are provided at: The patient’s place of residence, whether that is their personal home, family member’s home, assisted living facility, or nursing facility. It’s about how you LIVE.

For those patients who qualify, hospice provides: : 

For those patients who qualify, hospice provides: Physician services; Nursing care; Medical Equipment Medical Supplies; Drugs for Symptom Control and Pain Relief; Short-term Hospital Care; HHA & Homemaker Services; PT, OT, ST; Social Services; Spiritual Counseling; Dietary Counseling; Grief counseling for family for 13 months after date of death. It’s about how you LIVE.

In the home and nursing home we provide: : 

In the home and nursing home we provide: nursing visits scheduled according to each patient’s needs; care based on each patient’s and family’s needs and desires continuous care at end of life; the full array of services. It’s about how you LIVE.

Medication Benefit: : 

Medication Benefit: Hospice covers medications related to the primary terminal illness. The cost to the patient/family is 5% for each covered prescription up to $5.00. Usually, this is an enormous benefit - both financially and emotionally - to each patient and family. It’s about how you LIVE.

Medications Provided : 

Medications Provided Hospice Illness Pain Management; Depression; Anxiety; Nausea/Vomiting; Constipation; Diarrhea; Insomnia. It’s about how you LIVE.

Medical Equipment Benefit: : 

Medical Equipment Benefit: For hospice patients, there is no wait for medical equipment. As soon as the need for equipment is determined by criteria designed by the hospice team, the equipment is ordered and delivered - usually within 2-3 hours. It’s about how you LIVE.

Other Supplies Provided: : 

Other Supplies Provided: Up to 3 cans of nutritional supplement/day; Incontinent care supplies - 4 packages of briefs, 40 blue pads, and 2 packages of wipes/month. ~ These supplies are provided to all hospice patients requiring them regardless of payor source or living arrangement. Non-Covered Services: : 

Non-Covered Services: Services provided by more than one hospice provider (duplication of services); Room and board charges - for all cases other than short-term hospitalization for acute management of symptoms. It’s about how you LIVE.

Services Not Routinely Provided: : 

Services Not Routinely Provided: IV Therapy; Gastric Tube Feedings. ~ Each patient is evaluated on an individual basis to determine if suitable for hospice and the rationale for these services. At times, patients requiring these services will be admitted to hospice. ~ It’s about how you LIVE.

Basic Hospice Principles : 

Basic Hospice Principles Neither hasten nor postpone death Treat the patient and family Wholistic Abundant Life Hospice adds the component of “aggressive” palliative care.

“Allow Natural Death” : 

“Allow Natural Death” Simply means the medical community will work to cooperate with the patient’s body as he or she begins to die. Do Not Resuscitate means the same thing; however, “allow natural death” brings meaning to the tender side of hospice care and our expertise. It’s about how you LIVE.

Natural Dehydration : 

Natural Dehydration Cells undergo changes and release a natural anesthesia which promotes comfort. The patient experiences less pain as the body prepares for end-of-life. It’s about how you LIVE.

Hydration at End-of-Life : 

Hydration at End-of-Life Reversal of natural dehydration causes increased pain for a longer period of time. It’s about how you LIVE.

So, do we just “not care” about dehydration? : 

So, do we just “not care” about dehydration? Absolutely not. It is because we care so very much that we make no attempt to re-hydrate the patient who is experiencing natural dehydration related to end-of-life. We work to allow a natural death while promoting the utmost comfort in our patients. It’s about how you LIVE.

Pain and Symptom Management : 

Pain and Symptom Management Non-invasive and non-intrusive; Provided in a way that is easy for family members to manage; Oral for as long as possible - usually to the time of death; Suctioning, IV therapy and other invasive procedures as minimally as possible. It’s about how you LIVE.

Non-Invasive & Non-Intrusive : 

Non-Invasive & Non-Intrusive This means that Abundant Life Hospice provides care to patients and families in a manner that allows family life to continue as normally as possible for long as possible. The many high-tech devices that we have access to are wonderful - but can add much stress to an already overwhelmed caregiver. It’s about how you LIVE.

Grief Support Counseling : 

Grief Support Counseling Provided to family of the patient for 13 months after date of death. Telephone calls; Letters and cards, Personal visits and assessments; Support groups. Remember that at risk during this time is a newly widowed surviving spouse. It’s about how you LIVE.

An ongoing journey. : 

An ongoing journey. Hospice bereavement recognizes that each person experience grief in his/her own time. The goal of bereavement is not to help our families get over the loss, but to get them through it. It’s about how you LIVE.

Summary : 

Summary Hospice services are appropriate for patients who: have a life-limiting or terminal illness; Have a chronic illness that is not responding to treatment; And/Or: have determined they want no further aggressive or curative treatment. It’s about how you LIVE.

Referral to Hospice : 

Referral to Hospice Should be made as soon as the determination is made that the patient is diagnosed with: Life-limiting or terminal illness, Chronic illness now unresponsive to treatment, And/Or: The patient has made determination to seek no further aggressive or curative treatment. The sooner the referral is made, the sooner the patient and family can begin working through their grief and loss. It’s about how you LIVE.

Keep It Simple : 

Keep It Simple Ask yourself: Would the death of this person surprise me if it happened within the next year? If the answer is no, now is the time to begin talking about hospice. It’s about how you LIVE.

Consider Hospice Referral for Patients who: : 

Consider Hospice Referral for Patients who: Have unexplained weight loss; Multiple hospitalizations within 6 months for chronic illness; Chronic illness unresponsive to treatment; Loss of quality of life; Voice a desire for comfort care only and/or have Living Will. Also remember to consider hospice for those who: Have medical orders that read: Do Not Hospitalize or Comfort Measures Only It’s about how you LIVE.
