Disaster 2 - 30 Sec Blast All Clear 1 – 10 Sec Blast Localized Work Site: Regroup at the designated assemble point or other point as designated on your JSEA, if designated area is not safe, regroup at Security Gate Site-Wide Evacuation: You will be released from site by your supervisor Tornado/Hurricane Response: Tornado Watch: Site Supervision will be notified to have crews on alert, but site will not be evacuated Tornado Warning: Site will be evacuated – personnel to take most direct route off site while remaining out of path of tornado Imminent Tornado: Seek shelter in low area away from trailers. Lightning: Lightning w/in 12 miles = shutdown in an orderly manner and seek shelter until 30 minutes after last lightning strike. Emergency Response Procedures 4General Site Rules: All employees and visitors are required to follow all site safety and health rules at all times. The visitor site orientation only allows visitors onsite while being escorted by someone who has completed the employee site orientation. This Sight Specific Orientation is designed for all Contractor Employees. Minimum P.P.E. will be worn at all times while on the jobsite: Hardhat, Safety G lasses Marked with Z87, ankle high leather safety-toed boots, shirts with a minimum 4 ½” sleeves. Shorts will not be allowed at any time, as well as clothing which may be considered “offensive or improper” for a working environment. Administration Office Personnel are allowed a Variance in Foot Wear, Footwear should include a covered toe. No Open Toe Shoe, No heals greater than 2” (two inch). General Site Rules 5General Site Rules: Employees will park P.O.V.’s (Privately Owned Vehicles) in the Employee Parking Lot, outside the Plant. No Personal Vehicles are allowed inside the Plant without a copy of Insurance requirements, Rental Agreement with Contractor/Sub-Contractor on file with Pandora Safety. A Permit may be required to Drive a Vehicle into the Facility. Contractors and Sub-Contractors may need a Pandora issued parking tag. Only company vehicles should be driven into the designated job site and the company vehicles should be clearly marked with the Company Name displayed on the doors, no smaller that 4” x 8 ”. NOTE: FRC’s (Fire Retardant Clothing) will be required and Notified to employees at a later date during the turnaround by Supervision. General Site Rules 6General Site Rules: Possession of or use of alcohol, drugs, weapons, or explosives is strictly prohibited. Violation of this will result in immediate removal from the project. Smoking and Smokeless Tobacco is prohibited in all buildings and Units and will only be permitted in authorized areas. Cigarette ends (butts) will be disposed of in receptacles and not thrown on the ground. All trash will be placed in proper containers and not thrown on the ground. Construction activities are limited to the designated construction boundaries and will not go off these boundaries. Pets are not allowed on the construction site. All employees involved in significant At-Fault Incidents will require a post incident drug screen before they can return to the site. All near miss/hits and injuries must be reported immediately to their immediate supervisor or to the Safety Department . MAINTAIN PROFESSIONALISM AND RESPECT FOR PEOPLE AT ALL TIMES. General Site Rules 7Environmental Concerns: Notify your Supervisor and the Site Safety Manager of any spills or other Environmental situations, excavating or potential possibilities . ALWAYS check the MSDS of any Chemical in the area, know what you are dealing with. If Possible, stop the Leak or Place Barriers to prevent from spreading. Quality Management Policy 10 Pandora Project Priorities: Priorities RESPECT Client/Owner RESPECT Local C ommunity RESPECT Co-Workers Pandora Project Priorities 11STOP Work Authority: All employees have the authority and obligation to stop any task or operation where there are concerns or questions regarding the control of safety, health, and/or environmental risk. No work will resume until all STOP work issues and concerns have been adequately addressed and reported to Pandora Site Safety. There will be no form of retribution or intimidation for any person or company exercising their authority, as outlined in the STOP program. STOP Work Authority 12Incident Notification: ALL incidents including first aid, property damage, and near misses must be reported to your supervisor immediately! Notify your supervisor immediately via telephone, radio, or in person (no matter how minor). Notification must also be made to Safety as soon as possible. Incident Notification 13Incident Investigations: All incidents on the project will be investigated to prevent reoccurrence. The severity of the incident will determine the level of investigation. The purpose of the investigation is to find the cause, not fault. Incident Investigations 14Drug & Substance Abuse Policy: Pandora prohibits the unlawful use, sale, attempted sale, or purchase of illegal drugs while employed with Pandora or any company contracted to perform work with Pandora. The use of drugs and alcohol or being under the influence of drugs or alcohol is strictly prohibited for any Pandora job function. Drug & Substance Abuse Policy 15Drug & Substance Abuse Program: Pre-Employment Job candidates (including rehires) must submit to and pass a drug test as a qualification for and condition of employment with Pandora after an offer has been made. A confirmed positive result for drug substances tested will disqualify the person for employment. Pandora will withdraw its offer of employment to any job candidate who has a positive confirmatory test or who refuses to submit to drug testing. Reasonable Suspicion and Post Incident Pandora may require an employee to submit to drug and/or alcohol testing based upon evidence that the employee is using or has used alcohol or drugs in violation of this policy. Decisions to test must be made by at least two management representatives (i.e., salaried supervisor, manager) who have received training in accordance with this policy. Any employee associated with the cause or possible cause of an injury, near miss/hit, or property damage may require post-incident testing. Drug & Substance Abuse Program 16 Driving Policy: Pandora Maintains 4 Levels of Driver Status No Valid Drivers License / No Driving Allowed No Personal Vehicle with personal Insurance only and No Contract are allowed inside the Plant Personnel shall wear a seat belt at all times while traveling in a motor vehicle. Only Drivers of Vehicle in DOT Regulated Activities with all Appropriate Credentials Shall drive such Vehicle. Know what materials you are hauling Combined fuel capacity of portable tanks must not exceed 119 gallons Combined weight of hazardous materials must be under 440 lbs.. Any amount over these, is considered DOT Regulated Driving Policy 17 Driving Policy: Site access road speed limit is 15 mph All other roads will have a safe driving speed of 5 mph. Laydown Yard and active work areas are 5 mph. While vehicle is in motion, the use of phones, radios, or other communication devices is strictly prohibited. Anyone who drives a vehicle or piece of equipment off road without authorization is subject to disciplinary action up to and including removal from the site. STAY ON THE ROADS! Riding in the back of trucks (including pick-up trucks) is prohibited. Driving Policy 18Personal Protective Equipment (PPE): Hard Hats Required in all areas, unless otherwise posted (ANSI Z89) Bill forward, All Personnel to have their Name on there Hard H at Safety-toed Work Boots ANSI Z41 Long Pants Ear Plugs Noise levels over 85 dBA based on a 8hour TWA Safety Glasses ANSI Z87 Additional PPE may be required, as determined, during the JSEA development process – face shields, Goggles, Welding H oods, etc. All Safety Glasses MUST BE STAMPED Z-87 (if no visible stamp is on the Frame/Lens, the eye ware is PROHIBITED) Personal Protective Equipment (PPE) 19PPE: Contractors / Subcontractors will provide PPE to their employees. For all Pandora employees: Additional PPE will be available at the Tool Room or Safety Office. When P.P.E. is not needed any longer, it will be cleaned and returned to the Tool Room or Safety Office . All damaged or excessively worn PPE needs to be brought in to the Tool Room or Safety Office and replaced. When the possibility of lacerations to the hands is present, gloves must be worn (very few situations would allow not using gloves and must be defined on JSEA). PPE 20Insects and Plants: The following snakes, insects and plants may be encountered on the project site: Ticks, wasps, bees. Mosquitos Black Widow Spiders Brown Recluse Spiders Poisons from some plants will cause rashes and burns Alligators Various Snakes are possible Insects and Plants 21Avoiding Contact with Snakes, Insects, and Poisonous Plants: Be aware of your surroundings. Do not reach into areas that you cannot see. If you see an item of concern – do not approach it. Please do report it to your supervisor or Safety immediately. Keep your skin covered with pants, socks, shirt sleeves and gloves. Remember that even non-venomous snakes and insects can cause infections via bacteria or viruses. Use insect repellant to minimize exposure to ticks, chiggers, biting flies, and mosquitoes Avoiding Contact with Snakes, Insects, and Poisonous Plants 22In Case of Animal or Insect Bites: Identify the type of animal or insect if possible – do not approach the animal, snake or spider to do so. Report the bite immediately – get appropriate care. Care for venomous bites or stings Seek emergency medical services immediately. Remove any constricting items that the person may be wearing. Keep the person who was bit calm and minimize their activity level. Keep the bite area below the level of the heart to minimize return blood flow from the bite area to the rest of the body. Keep the bite area clean and covered. In Case of Animal or Insect Bites 23Safety Discipline Program: Fairly applied at all employment levels on this project site Zero tolerance for blatant, willful violations of company policies regarding certain aspects of: fall protection excavation and trenching lockout/tagout (LOTO) confined space entry motor vehicle operation crane and erection safety working near energized power lines without proper safeguarding; and drugs and/or alcohol abuse. Safety Discipline Program 24Barricades: Barricade Types Yellow or Yellow/Black tape is used to convey “ CAUTION ” Personnel are allowed to enter the barricade only after they have read the tag and have fulfilled the requirements of the JSEA. Red tape is used to convey “ DANGER – DO NOT ENTER ” Personnel are not allowed to enter without the approval of the foreman/craftsman in charge. Must wear all required PPE indicated for the hazard and fulfill the requirements of the JSEA. Barricade Tags Tags shall be placed on all barricades to indicate: Supervisor who placed the barricade tape Purpose or reason for the barricade What PPE is required to enter Date placed No crossing into a RED barricaded area without the following: Obtaining permission from the individual who installed the barricade tape Reviewing and signing of the JSEA with a crew member Crossing a RED Barricade without Permission, will lead to severe discipline, and or termination of employment. Barricades 25Training: If you have not been trained to do a task or use a piece of equipment or tool, use your STOP Work Authority. If you have been asked to perform a task which you have not been trained on, notify your supervisor and use your STOP Work Authority. Anyone who operates equipment is required to be licensed and to have in their possession the required certification cards. Anyone who is required to work on a Scaffold, is requires to be Scaffold User Trained. Anyone who is required to enter a Confined Space is required to be Confined Space Trained. Anyone who is required to wear a Respirator or Supplied Air is required to have a Current PFT (Pulmonary Fit Test) and be Fitted for the appropriate size and manufacture of equipment used. Do not allow anyone on site to perform a task for which they have not been trained. See the Pandora site Safety Manual for additional requirements. Training 26Work Permits: Before conducting work, a Permit will be required, Some TASK may require additional Permits that involves confined space entry, work on energy systems, ground disturbance in locations where buried hazards may exist, or hot work in potentially explosive environments, a permit must be obtained that: Defines the scope of work Identifies hazards and assesses risk Establishes control measures to eliminate or mitigate hazards Links the work to other associated work permits or simultaneous operations Is authorized by the responsible person(s) Communicates above information to all involved in the work Ensures adequate control over the return to normal operations Permit will be administered by an authorized Pandora Representative. Permits will be issued for one work shift, some exceptions may be obtained in writing. If conditions change, the permit is void and a new permit must be issued. Work Permits 27What is a Job Safety Environmental Analysis (JSEA)?: A documented process where tasks are outlined step-by-step. The actual or predicted hazards of each step are defined in writing. The required controls and/or precautions are defined for each hazard listed. The plan is modified, where necessary, to eliminate or control the defined hazards. The plan is reviewed by site management throughout the day for opportunities to improve. The plan is reviewed at the end of a shift for improvements. The JSEA is Closed at the end of the TASK by the Leadman signing off that the Housekeeping is Complete and No incidents/injuries occurred. What is a Job Safety Environmental Analysis (JSEA)? 28What is a JSEA? (cont.): Begins at the office or check-in location All crew members will participate in the development. Will be completed at the job site (No pre-printed JSEA’s) Any individual visiting the work location will be briefed by a crew member and required to sign the JSEA. (Meet & Greet) If work site conditions change, the JSEA will need to be updated and re-signed. The JSEA must be kept at the work site for the duration of the task. The JSEA is only valid for a maximum of one work shift. Multi-day or shift tasks should use the previous day’s JSEA as a learning tool to transfer knowledge. What is a JSEA? (cont.) 29The JSEA Process: Planning A systematic workplan is developed step-by-step for activities. Potential hazards are identified. Corrective measures are then developed to prevent those hazards from occurring. Organization The JSEA helps identify the resources (time, tools, equipment, materials and personnel) needed to do the job correctly. This ensures the right resources (people, PPE, equipment, tools, etc.) are available and ready when needed. Sequencing The steps of each task can be sequenced to eliminate hazards as well as improve productivity. All JSEA’s are to be turned in to your Supervisor at the end of the Job Task for monitoring and Record Keeping requirements. Training All work team members associated with the task to be performed must help develop the JSEA. Consequently, everyone is made aware of the potential hazards and plans to deal with them before the job begins. Thus, the JSEA becomes a training tool as well as a work tool. The JSEA Process 30Lockout/Tagout (LOTO): LOTO procedures are required to be followed when working on equipment that has the potential to release stored energy such as electrical, hydraulic, pneumatic, mechanical, chemical, thermal and radiation. Requires that a designated individual turns off and disconnects the machinery or equipment from its energy source(s) before performing service or maintenance and that the authorized employee(s) either lock or tag the energy-isolating device(s) to prevent the release of hazardous energy and take steps to verify that the energy has been isolated effectively. A person performing LOTO must be designated as authorized. LOTO sequence to ensure safe energy isolation: Identify all energy sources Notify all affected employees Isolate energy sources Control stored energy by bleeding off, draining etc. Apply LOTO devices to switches, circuit breakers, etc. Verify LOTO by trying buttons and switches LOTO removal = Reverse of Application Lockout/Tagout (LOTO) 31Hazard Communication Program: Training Everyone on site will receive training from his/her employer prior to working on the project. Everyone WILL be properly trained to perform the duties Everyone on site will receive additional or receive re-training if chemical or process changes are involved. Material Safety Data Sheets (MSDS) Copies of MSDS will be available on site. Contactors are to provide copies of MSDS to Pandora Site Safety Management. Labeling All containers above household use shall be labeled. Any product transferred from its original container shall be labeled using an approved Hazardous Material Identification System. Hazard Communication Program 32Material Safety Data Sheets (MSDS): Identifies hazards Informs personnel on site how to work safely with products Identifies what to do in an emergency Identifies What PPE must be worn while around/using the chemical Material Safety Data Sheets (MSDS) 33NFPA Key Points: Red - Fire and Explosion Blue - Health Hazards Yellow - Reactivity White - Other Information - corrosive Protective Equipment and Other Control Measures are listed with each Additional Precautions Listed NFPA Key Points 34Housekeeping: Leave the site and/or work area better than you found it Ensure construction waste and/or debris is in the appropriate container Ensure that tobacco waste is placed in the container inside the designated smoke area Keep Cords and Hoses from becoming Tripping Hazards Keep your work area FREE of debris and un-used equipment Housekeeping 35Confined Space Entry: Confined spaces can be extremely hazardous if procedures are not followed. Confined spaces will require an entry permit, or be reclassified as non-permit required, and comply with construction safety procedures. Permits will be issued by a Designated, Pandora Site Permit Writer. All Confined Spaces will require a Trained Hole Watch to be at the entry point of the Confined Space. All Entrants into a Confined Space will have been “Confined Space Trained” (other requirements may be required) See Pandora Site Safety Manual for additional requirements. Confined Space Entry 36Ground Fault Circuit Interrupter (GFCI): Pandora construction projects require GFCI protection on all electrical power tools and equipment. GFCIs and electrical lead cords shall be inspected Monthly and color coded by a competent person to ensure proper operation. Only heavy duty electrical cords of 12 ga. Double insulated or larger shall be used. Ground Fault Circuit Interrupter (GFCI) 37Lead Paint : Lead Paint Pandora Methanol and Ammonia Units are known to have Lead Paint present. Lead Paint abatement is in process at this time. If you or your co-workers have a task to perform and are not sure if the paint on the specific equipment is Lead Free, Check with your Supervisor, or ask Operations or the Pandora Safety if Lead Testing has been completed on the equipment. DO NOT grind, cut or chip on any paint without having verification from Operations the Paint is Lead Free. 38Asbestos: Asbestos Asbestos is in the Pandora and Ammonia Units. Asbestos is known to be a cause of pleural mesothelioma. Asbestos is being Abated in both, Pandora Methanol and Ammonia Units at this time. If you are working around any Insulation NOT CLEARLY IDENTIFIED as Non Asbestos, Notify your Supervisor or Pandora Safety before performing any task that may put you or co-workers at risk. Do Not Disturb any Insulation without prior approval from Operations. 39Ladder Safety: Ladder Types Type III Duty Rating: 200 lbs.. load capacity Type II Duty Rating: 225 lbs.. load capacity Type I Duty Rating: 250 lbs.. load capacity Type IA Duty Rating: 300 lbs.. load capacity Type 1AA Duty Rating: 375 lbs. load capacity Before using a ladder, be sure you are using the proper ladder for the task. Maintain three point contact. NO METAL or WOOD LADDERS ALLOWED Ladders must be secured and extend three feet above landing and be tied off to a secured point Ladder Safety 40 Hand Power Tools: All hand power tools shall be inspected prior to use and Monthly by a designated competent person Tools shall be used and maintained in accordance with manufacturers’ recommendations. Tools shall not be modified without written permission from the manufacturer. Only use tools in which you have been trained to use. Use the right tool for the job. No altered, homemade or broken tools shall be used Hand Power Tools 41Rigging and Slings: Rigging must be performed by certified rigging individuals. Rigger is responsible for keeping non-essential personnel out of the lift area and must not direct the lift over personnel. Personnel involved with rigging operations shall successfully complete a recognized training program. All loads shall be placed on blocks, and slings must never be pulled from under the load while the load is resting on the sling. Tag lines are required to be used on every load. The use of a Pipe or Pipes including Conduit is prohibited for the use of lifting, anchoring or using to place a lifting devise. Any persons involved with using a Pipe or Pipes including Conduit for tying a lifting devise will be subject to Termination of Employment. . Rigging and Slings 42Rigging and Slings: Slings shall be inspected daily before use. Slings shall be inspected and color coded by a qualified individual. Manufacture tags shall not be removed from slings. Slings shall not be used for any other application other than the lift. Slings that show signs of excessive broken wires, bird caging or core protrusion shall be removed from service. Slings with damaged hooks, latches, eyes or evidence of heat damage shall be removed from service. Slings shall be stored in a dry, clean contaminate-free environment. Care shall be taken with synthetic slings getting wet and around sharp edges Rigging and Slings 43Protection from Falling Objects: Limit what is taken to elevated locations. Use a barrier along platform or elevated floor railing where material is stored. Barricade the area below elevated work and keep personnel out of the barricaded area. Use flag‑persons/ground‑persons when barricading the area below is not practical. Use "lanyards" on tools being used over the sides of platforms or floors or over unprotected elevated areas. Use tool belts to hold tools when climbing at heights. Tools should fit securely in the tool belt. Tools must not be field-modified, even in order to keep them from falling. If you must work under others in elevated locations, then positive steps, shall be taken to protect those below. Protection from Falling Objects 44Specialized Operations: The following operations require detailed training, certification, and field performance evaluation by your employer, an Authorized Operator’s Card is required per OSHA. Boom/scissor elevated work platforms and lifts Crane-suspended work platforms Cranes/mobile cranes Critical lifts Earth moving equipment Fall protection Fire extinguishers Forklifts and heavy equipment Scaffolding Steel erection/disassembly Trenching and excavation Utility tie-in Working on or near energized electrical equipment Confined Space Work Breathing Air/ Fresh Air Work Fire Watch/Hole Watch Specialized Operations 45Boom/Scissor Elevated Work Platforms: All lifts must be inspected by a qualified person and occur upon arrival at the job site and quarterly or every 200 hours, whichever comes first. A competent operator must inspect all controls and safety devices each day prior to use. Daily inspections must be documented. All lift operators shall be trained by a designated competent person. A copy of their field evaluation and training records will be kept on site in the field office. The contractor must also issue proof of qualification to each operator. Operators must have this proof of qualification with them at all times while operating that equipment. Fall Protection is REQUIRED 100% of the time Boom/Scissor Elevated Work Platforms 46Crane-suspended Work Platforms: Crane-suspended work platforms to hoist or suspend employees are acceptable in unique work situations. The Contractor must develop and implement a program for control and authorization of crane-suspended work platforms. The Contractor Site Manager or qualified designee must approve using a crane-suspended work platform using the Crane Suspended Work Platform Permit. Crane-suspended Work Platforms 47Cranes: All cranes shall be inspected upon arrival to the site by a qualified individual. Cranes shall be inspected daily prior to use. All cranes shall be equipped with: Annual permits Operator’s manual Fire extinguisher Operator logs Signal charts Load charts Anemometer required at the block or the jib Cranes 48Mobile Cranes: Only personnel who have been designated as a Qualified Operator, by Pandora, may operate mobile cranes. Operators must be assessed on site prior to operating equipment. Operator shall have a valid driver’s license. Weather conditions may limit crane use. A spotter is required at all times. A barricade is required on swing radius of all cranes. LMI and anti-two block device must be functional. All lifts require a JSEA. See the Pandora Site Safety Manual for additional requirements. Mobile Cranes 49Crane Lifts: Crane Lift Planning Must be Completed For All Lifts Lifts will be categorized based on level of risk Level of risk is based on review of load weight, percent of chart capacity, replacement time, replacement cost, and number of cranes used in the lift. Approvals required will vary based on risk level. Documentation required will also vary based on risk level. See the Pandora Site Safety Manual for additional requirements. Crane Lifts 50Fall Protection: Fall protection is required when working more than six feet above a floor or platform surface, or within six feet of any unguarded roof or platform edge more than six feet above ground level. “ This is called the six foot rule. ” Appropriate guarding and warning devices (chain, rope, cable) must be in place to protect workers from open holes and leading edges. Fall Protection Equipment MUST be inspected and logged Monthly and be clearly tagged as inspected. Each Individual MUST inspect his/her Fall Protection Equipment before each use. Fall Protection 51Fall Protection (cont.): Contracting companies shall train employees to: Recognize the hazards of falling from heights and avoid falls to lower levels through holes or openings in walking/working surfaces and walls. Training programs shall include prevention, control, and fall-arrest systems. Employee’s shall be trained to properly tie-off 100% of the time Training programs will include proper use of Tool Lanyards or other means of securing hand tool to prevent tools from falling. See the Pandora Site Safety Manual for additional requirements Fall Protection (cont.) 52Fire Extinguishers: The Contractor shall plan for and supply adequate fire extinguishers for all work areas, spaces, floors, and buildings where burning, welding, cutting or open-flame equipment is used or where smoking is allowed. Each welder, burner, or operator of open-flame equipment shall have an individual fire extinguisher within immediate reach of 10 feet (3 meters). Fire extinguishers for work on scaffolds and elevated platforms shall be readily available (within reach) to the workers. Fire extinguishers are also required for fueling operations. Each piece of motorized equipment shall be equipped with a fire extinguisher. Fire Extinguishers 53Fire Extinguishers (cont.): Fire extinguishers must be routinely inspected at least monthly by a competent person. Each extinguisher must have a durable tag securely attached, showing the date of the last thorough inspection, maintenance or recharge date and the initials or signature of the person who performs this service. Anyone using fire extinguishers must ensure the marking system remains legible between inspections. It is recommended that all lead persons, foremen, and supervisors be provided with “Hands On” training. All fire watch personnel shall be provided with training for the fire extinguishers they might be expected to use. Fire Extinguishers (cont.) 54Forklifts and Heavy Equipment: Only certified operators are allowed to operate equipment on this site. Each different "Type" of equipment will require individual certification for its operation. Each forklift attachment will require certification for its operation. The operator must carry his/her operator's card whenever operating a forklift or other equipment. All operators must have a valid driver’s license. Forklifts and Heavy Equipment 55Trenching and Excavation: The contractor shall develop a plan for control of excavations. Before beginning an excavation, the contractor must take the following minimal precautions: Complete an Excavation and Trenching Permit Remove or stabilize all surface encumbrances that create hazards to employees such as brush, spoil dirt, or boulders Erect either warning or protective barricades Keep spoil dirt and any material or equipment that may fall into an excavation at least three feet (1 meter) from the edge Remove loose rock or soil that could fall from the face of the excavation Protect, support, or remove underground installations or adjacent structures, as necessary, to protect employees and the environment Trenching and Excavation 56Working on or Near Energized Electrical Equipment: Four types of hazards Shock and burn due to contact or flashover Electrical flash burn from arc Blast injury resulting from electrical arcs Upset or shutdown of an operating area The electrically hazardous task must be analyzed considering two primary hazards - flash hazard and shock hazard. These hazards are identified by using boundaries based on energy levels. Performing work inside these boundaries requires training, planning, procedures, and PPE. See the Pandora Site Safety Manual for additional requirements Working on or Near Energized Electrical Equipment 57Tower Rescue: In the event of a fall incident, regardless of the medical condition of the employee, the supervisor or crew lead will be notified immediately. Prior to starting work, determine what information you will need to contact rescue and other emergency services. List this information on your JSEA. In the event an injured worker is unable to remove himself/herself from the tower, rescue procedures have been developed that will be put into action to ensure the employee receives an adequate and timely rescue. Rescue and Tower Climbing Procedures will have to be reviewed for compatibility with each manufactures tower configuration. The minimum requirement is to have at least two trained rescuers and one set of rescue equipment available within a 5 minute response time from the base of the tower. Have your designated rescuers listed on your JSEA. Tower Rescue 58Tower Rescue (cont.): Be prepared with additional information when you call for rescue or emergency medical service support. Examples of this need would be, but are not limited to: Site location Location of employee within the tower Condition of employee needing rescue / Incapacitated employee Ground assistance available Is there any additional rescue equipment needed? If first aid can be given before the injured employee is rescued from the tower, the rescuer should provide first aid before rescuing the employee. First aid should be administered to the injured employee by a trained employee until the local emergency medical team arrives. Tower Rescue (cont.) 59Hot Weather: Care must be taken to avoid any heat related illnesses such as heat stress, heat exhaustion or heat stroke. Make sure you are getting adequate rest before coming to work so that your body is ready for the work at hand. Acclimate new employees to working in extreme weather conditions. Stay hydrated throughout the day. Drink before you are thirsty. In extreme heat conditions, modify your work schedule as necessary to protect the employees. Hot Weather 60Cold Weather: When temperatures of extreme cold are expected, management will decide if work will take place. This will mostly come from the wind chill that will be created by high winds. Protect yourself from the cold, Hypothermia is a very constant and real danger on the job site. Cold Weather 61Contractor / Sub-Contractor Parking: All Employees will receive Parking instructions at the end of this Pandora Orientation. Bus Loading and Un-Loading shall be in a orderly fashion. Any Dis-Orderly Conduct may result in termination. Craft Personnel are Prohibited from parking in the Management Parking Lot. Emergency Response Procedures 4General Site Rules: All employees and visitors are required to follow all site safety and health rules at all times. The visitor site orientation only allows visitors onsite while being escorted by someone who has completed the employee site orientation. This Sight Specific Orientation is designed for all Contractor Employees. Minimum P.P.E. will be worn at all times while on the jobsite: Hardhat, Safety G lasses Marked with Z87, ankle high leather safety-toed boots, shirts with a minimum 4 ½” sleeves. Shorts will not be allowed at any time, as well as clothing which may be considered “offensive or improper” for a working environment. Administration Office Personnel are allowed a Variance in Foot Wear, Footwear should include a covered toe. No Open Toe Shoe, No heals greater than 2” (two inch). General Site Rules 5General Site Rules: Employees will park P.O.V.’s (Privately Owned Vehicles) in the Employee Parking Lot, outside the Plant. No Personal Vehicles are allowed inside the Plant without a copy of Insurance requirements, Rental Agreement with Contractor/Sub-Contractor on file with Pandora Safety. A Permit may be required to Drive a Vehicle into the Facility. Contractors and Sub-Contractors may need a Pandora issued parking tag. Only company vehicles should be driven into the designated job site and the company vehicles should be clearly marked with the Company Name displayed on the doors, no smaller that 4” x 8 ”. NOTE: FRC’s (Fire Retardant Clothing) will be required and Notified to employees at a later date during the turnaround by Supervision. General Site Rules 6General Site Rules: Possession of or use of alcohol, drugs, weapons, or explosives is strictly prohibited. Violation of this will result in immediate removal from the project. Smoking and Smokeless Tobacco is prohibited in all buildings and Units and will only be permitted in authorized areas. Cigarette ends (butts) will be disposed of in receptacles and not thrown on the ground. All trash will be placed in proper containers and not thrown on the ground. Construction activities are limited to the designated construction boundaries and will not go off these boundaries. Pets are not allowed on the construction site. All employees involved in significant At-Fault Incidents will require a post incident drug screen before they can return to the site. All near miss/hits and injuries must be reported immediately to their immediate supervisor or to the Safety Department . MAINTAIN PROFESSIONALISM AND RESPECT FOR PEOPLE AT ALL TIMES. No work will resume until all STOP work issues and concerns have been adequately addressed and reported to Pandora Site Safety. There will be no form of retribution or intimidation for any person or company exercising their authority, as outlined in the STOP program. STOP Work Authority 12Incident Notification: ALL incidents including first aid, property damage, and near misses must be reported to your supervisor immediately! Notify your supervisor immediately via telephone, radio, or in person (no matter how minor). Notification must also be made to Safety as soon as possible. Incident Notification 13Incident Investigations: All incidents on the project will be investigated to prevent reoccurrence. The severity of the incident will determine the level of investigation. The purpose of the investigation is to find the cause, not fault. Incident Investigations 14Drug & Substance Abuse Policy: Pandora prohibits the unlawful use, sale, attempted sale, or purchase of illegal drugs while employed with Pandora or any company contracted to perform work with Pandora. The use of drugs and alcohol or being under the influence of drugs or alcohol is strictly prohibited for any Pandora job function. Drug & Substance Abuse Policy 15Drug & Substance Abuse Program: Pre-Employment Job candidates (including rehires) must submit to and pass a drug test as a qualification for and condition of employment with Pandora after an offer has been made. A confirmed positive result for drug substances tested will disqualify the person for employment. Pandora will withdraw its offer of employment to any job candidate who has a positive confirmatory test or who refuses to submit to drug testing. Reasonable Suspicion and Post Incident Pandora may require an employee to submit to drug and/or alcohol testing based upon evidence that the employee is using or has used alcohol or drugs in violation of this policy. Decisions to test must be made by at least two management representatives (i.e., salaried supervisor, manager) who have received training in accordance with this policy. Any employee associated with the cause or possible cause of an injury, near miss/hit, or property damage may require post-incident testing. Drug & Substance Abuse Program 16 Driving Policy: Pandora Maintains 4 Levels of Driver Status No Valid Drivers License / No Driving Allowed No Personal Vehicle with personal Insurance only and No Contract are allowed inside the Plant Personnel shall wear a seat belt at all times while traveling in a motor vehicle. Only Drivers of Vehicle in DOT Regulated Activities with all Appropriate Credentials Shall drive such Vehicle. Know what materials you are hauling Combined fuel capacity of portable tanks must not exceed 119 gallons Combined weight of hazardous materials must be under 440 lbs.. Any amount over these, is considered DOT Regulated Driving Policy 17 Driving Policy: Site access road speed limit is 15 mph All other roads will have a safe driving speed of 5 mph. Laydown Yard and active work areas are 5 mph. While vehicle is in motion, the use of phones, radios, or other communication devices is strictly prohibited. Anyone who drives a vehicle or piece of equipment off road without authorization is subject to disciplinary action up to and including removal from the site. STAY ON THE ROADS! Riding in the back of trucks (including pick-up trucks) is prohibited. Driving Policy 18Personal Protective Equipment (PPE): Hard Hats Required in all areas, unless otherwise posted (ANSI Z89) Bill forward, All Personnel to have their Name on there Hard H at Safety-toed Work Boots ANSI Z41 Long Pants Ear Plugs Noise levels over 85 dBA based on a 8hour TWA Safety Glasses ANSI Z87 Additional PPE may be required, as determined, during the JSEA development process – face shields, Goggles, Welding H oods, etc. All Safety Glasses MUST BE STAMPED Z-87 (if no visible stamp is on the Frame/Lens, the eye ware is PROHIBITED) Personal Protective Equipment (PPE) 19PPE: Contractors / Subcontractors will provide PPE to their employees. For all Pandora employees: Additional PPE will be available at the Tool Room or Safety Office. When P.P.E. is not needed any longer, it will be cleaned and returned to the Tool Room or Safety Office . All damaged or excessively worn PPE needs to be brought in to the Tool Room or Safety Office and replaced. When the possibility of lacerations to the hands is present, gloves must be worn (very few situations would allow not using gloves and must be defined on JSEA). PPE 20Insects and Plants: The following snakes, insects and plants may be encountered on the project site: Ticks, wasps, bees. Mosquitos Black Widow Spiders Brown Recluse Spiders Poisons from some plants will cause rashes and burns Alligators Various Snakes are possible Insects and Plants 21Avoiding Contact with Snakes, Insects, and Poisonous Plants: Be aware of your surroundings. Do not reach into areas that you cannot see. If you see an item of concern – do not approach it. Please do report it to your supervisor or Safety immediately. Keep your skin covered with pants, socks, shirt sleeves and gloves. Remember that even non-venomous snakes and insects can cause infections via bacteria or viruses. Use insect repellant to minimize exposure to ticks, chiggers, biting flies, and mosquitoes Avoiding Contact with Snakes, Insects, and Poisonous Plants 22In Case of Animal or Insect Bites: Identify the type of animal or insect if possible – do not approach the animal, snake or spider to do so. Report the bite immediately – get appropriate care. Care for venomous bites or stings Seek emergency medical services immediately. Remove any constricting items that the person may be wearing. Keep the person who was bit calm and minimize their activity level. Keep the bite area below the level of the heart to minimize return blood flow from the bite area to the rest of the body. Keep the bite area clean and covered. In Case of Animal or Insect Bites 23Safety Discipline Program: Fairly applied at all employment levels on this project site Zero tolerance for blatant, willful violations of company policies regarding certain aspects of: fall protection excavation and trenching lockout/tagout (LOTO) confined space entry motor vehicle operation crane and erection safety working near energized power lines without proper safeguarding; and drugs and/or alcohol abuse. Safety Discipline Program 24Barricades: Barricade Types Yellow or Yellow/Black tape is used to convey “ CAUTION ” Personnel are allowed to enter the barricade only after they have read the tag and have fulfilled the requirements of the JSEA. Red tape is used to convey “ DANGER – DO NOT ENTER ” Personnel are not allowed to enter without the approval of the foreman/craftsman in charge. Must wear all required PPE indicated for the hazard and fulfill the requirements of the JSEA. Barricade Tags Tags shall be placed on all barricades to indicate: Supervisor who placed the barricade tape Purpose or reason for the barricade What PPE is required to enter Date placed No crossing into a RED barricaded area without the following: Obtaining permission from the individual who installed the barricade tape Reviewing and signing of the JSEA with a crew member Crossing a RED Barricade without Permission, will lead to severe discipline, and or termination of employment. Barricades 25Training: If you have not been trained to do a task or use a piece of equipment or tool, use your STOP Work Authority. If you have been asked to perform a task which you have not been trained on, notify your supervisor and use your STOP Work Authority. Anyone who operates equipment is required to be licensed and to have in their possession the required certification cards. Anyone who is required to work on a Scaffold, is requires to be Scaffold User Trained. Anyone who is required to enter a Confined Space is required to be Confined Space Trained. Anyone who is required to wear a Respirator or Supplied Air is required to have a Current PFT (Pulmonary Fit Test) and be Fitted for the appropriate size and manufacture of equipment used. Do not allow anyone on site to perform a task for which they have not been trained. See the Pandora site Safety Manual for additional requirements. Training 26Work Permits: Before conducting work, a Permit will be required, Some TASK may require additional Permits that involves confined space entry, work on energy systems, ground disturbance in locations where buried hazards may exist, or hot work in potentially explosive environments, a permit must be obtained that: Defines the scope of work Identifies hazards and assesses risk Establishes control measures to eliminate or mitigate hazards Links the work to other associated work permits or simultaneous operations Is authorized by the responsible person(s) Communicates above information to all involved in the work Ensures adequate control over the return to normal operations Permit will be administered by an authorized Pandora Representative. Permits will be issued for one work shift, some exceptions may be obtained in writing. If conditions change, the permit is void and a new permit must be issued. Work Permits 27What is a Job Safety Environmental Analysis (JSEA)?: A documented process where tasks are outlined step-by-step. The actual or predicted hazards of each step are defined in writing. The required controls and/or precautions are defined for each hazard listed. The plan is modified, where necessary, to eliminate or control the defined hazards. The plan is reviewed by site management throughout the day for opportunities to improve. The plan is reviewed at the end of a shift for improvements. The JSEA is Closed at the end of the TASK by the Leadman signing off that the Housekeeping is Complete and No incidents/injuries occurred. What is a Job Safety Environmental Analysis (JSEA)? 28What is a JSEA? (cont.): Begins at the office or check-in location All crew members will participate in the development. Will be completed at the job site (No pre-printed JSEA’s) Any individual visiting the work location will be briefed by a crew member and required to sign the JSEA. (Meet & Greet) If work site conditions change, the JSEA will need to be updated and re-signed. The JSEA must be kept at the work site for the duration of the task. The JSEA is only valid for a maximum of one work shift. Multi-day or shift tasks should use the previous day’s JSEA as a learning tool to transfer knowledge. What is a JSEA? (cont.) 29The JSEA Process: Planning A systematic workplan is developed step-by-step for activities. Potential hazards are identified. Corrective measures are then developed to prevent those hazards from occurring. Organization The JSEA helps identify the resources (time, tools, equipment, materials and personnel) needed to do the job correctly. This ensures the right resources (people, PPE, equipment, tools, etc.) are available and ready when needed. Sequencing The steps of each task can be sequenced to eliminate hazards as well as improve productivity. All JSEA’s are to be turned in to your Supervisor at the end of the Job Task for monitoring and Record Keeping requirements. Training All work team members associated with the task to be performed must help develop the JSEA. Consequently, everyone is made aware of the potential hazards and plans to deal with them before the job begins. Thus, the JSEA becomes a training tool as well as a work tool. The JSEA Process 30Lockout/Tagout (LOTO): LOTO procedures are required to be followed when working on equipment that has the potential to release stored energy such as electrical, hydraulic, pneumatic, mechanical, chemical, thermal and radiation. Requires that a designated individual turns off and disconnects the machinery or equipment from its energy source(s) before performing service or maintenance and that the authorized employee(s) either lock or tag the energy-isolating device(s) to prevent the release of hazardous energy and take steps to verify that the energy has been isolated effectively. A person performing LOTO must be designated as authorized. LOTO sequence to ensure safe energy isolation: Identify all energy sources Notify all affected employees Isolate energy sources Control stored energy by bleeding off, draining etc. Apply LOTO devices to switches, circuit breakers, etc. Verify LOTO by trying buttons and switches LOTO removal = Reverse of Application Lockout/Tagout (LOTO) 31Hazard Communication Program: Training Everyone on site will receive training from his/her employer prior to working on the project. Everyone WILL be properly trained to perform the duties Everyone on site will receive additional or receive re-training if chemical or process changes are involved. Material Safety Data Sheets (MSDS) Copies of MSDS will be available on site. Contactors are to provide copies of MSDS to Pandora Site Safety Management. Labeling All containers above household use shall be labeled. Any product transferred from its original container shall be labeled using an approved Hazardous Material Identification System. Hazard Communication Program 32Material Safety Data Sheets (MSDS): Identifies hazards Informs personnel on site how to work safely with products Identifies what to do in an emergency Identifies What PPE must be worn while around/using the chemical Material Safety Data Sheets (MSDS) 33NFPA Key Points: Red - Fire and Explosion Blue - Health Hazards Yellow - Reactivity White - Other Information - corrosive Protective Equipment and Other Control Measures are listed with each Additional Precautions Listed NFPA Key Points 34Housekeeping: Leave the site and/or work area better than you found it Ensure construction waste and/or debris is in the appropriate container Ensure that tobacco waste is placed in the container inside the designated smoke area Keep Cords and Hoses from becoming Tripping Hazards Keep your work area FREE of debris and un-used equipment Housekeeping 35Confined Space Entry: Confined spaces can be extremely hazardous if procedures are not followed. Confined spaces will require an entry permit, or be reclassified as non-permit required, and comply with construction safety procedures. Permits will be issued by a Designated, Pandora Site Permit Writer. All Confined Spaces will require a Trained Hole Watch to be at the entry point of the Confined Space. All Entrants into a Confined Space will have been “Confined Space Trained” (other requirements may be required) See Pandora Site Safety Manual for additional requirements. Confined Space Entry 36Ground Fault Circuit Interrupter (GFCI): Pandora construction projects require GFCI protection on all electrical power tools and equipment. GFCIs and electrical lead cords shall be inspected Monthly and color coded by a competent person to ensure proper operation. Only heavy duty electrical cords of 12 ga. Double insulated or larger shall be used. Ground Fault Circuit Interrupter (GFCI) 37Lead Paint : Lead Paint Pandora Methanol and Ammonia Units are known to have Lead Paint present. Lead Paint abatement is in process at this time. If you or your co-workers have a task to perform and are not sure if the paint on the specific equipment is Lead Free, Check with your Supervisor, or ask Operations or the Pandora Safety if Lead Testing has been completed on the equipment. DO NOT grind, cut or chip on any paint without having verification from Operations the Paint is Lead Free. 38Asbestos: Asbestos Asbestos is in the Pandora and Ammonia Units. Asbestos is known to be a cause of pleural mesothelioma. Asbestos is being Abated in both, Pandora Methanol and Ammonia Units at this time. If you are working around any Insulation NOT CLEARLY IDENTIFIED as Non Asbestos, Notify your Supervisor or Pandora Safety before performing any task that may put you or co-workers at risk. Do Not Disturb any Insulation without prior approval from Operations. 39Ladder Safety: Ladder Types Type III Duty Rating: 200 lbs.. load capacity Type II Duty Rating: 225 lbs.. load capacity Type I Duty Rating: 250 lbs.. load capacity Type IA Duty Rating: 300 lbs.. load capacity Type 1AA Duty Rating: 375 lbs. load capacity Before using a ladder, be sure you are using the proper ladder for the task. Maintain three point contact. NO METAL or WOOD LADDERS ALLOWED Ladders must be secured and extend three feet above landing and be tied off to a secured point Ladder Safety 40 Hand Power Tools: All hand power tools shall be inspected prior to use and Monthly by a designated competent person Tools shall be used and maintained in accordance with manufacturers’ recommendations. Tools shall not be modified without written permission from the manufacturer. Only use tools in which you have been trained to use. Use the right tool for the job. No altered, homemade or broken tools shall be used Hand Power Tools 41Rigging and Slings: Rigging must be performed by certified rigging individuals. Rigger is responsible for keeping non-essential personnel out of the lift area and must not direct the lift over personnel. Personnel involved with rigging operations shall successfully complete a recognized training program. All loads shall be placed on blocks, and slings must never be pulled from under the load while the load is resting on the sling. Tag lines are required to be used on every load. The use of a Pipe or Pipes including Conduit is prohibited for the use of lifting, anchoring or using to place a lifting devise. Any persons involved with using a Pipe or Pipes including Conduit for tying a lifting devise will be subject to Termination of Employment. . Rigging and Slings 42Rigging and Slings: Slings shall be inspected daily before use. Slings shall be inspected and color coded by a qualified individual. Manufacture tags shall not be removed from slings. Slings shall not be used for any other application other than the lift. Slings that show signs of excessive broken wires, bird caging or core protrusion shall be removed from service. Slings with damaged hooks, latches, eyes or evidence of heat damage shall be removed from service. Slings shall be stored in a dry, clean contaminate-free environment. Care shall be taken with synthetic slings getting wet and around sharp edges Rigging and Slings 43Protection from Falling Objects: Limit what is taken to elevated locations. Use a barrier along platform or elevated floor railing where material is stored. Barricade the area below elevated work and keep personnel out of the barricaded area. Use flag‑persons/ground‑persons when barricading the area below is not practical. Use "lanyards" on tools being used over the sides of platforms or floors or over unprotected elevated areas. Use tool belts to hold tools when climbing at heights. Tools should fit securely in the tool belt. Tools must not be field-modified, even in order to keep them from falling. If you must work under others in elevated locations, then positive steps, shall be taken to protect those below. Protection from Falling Objects 44Specialized Operations: The following operations require detailed training, certification, and field performance evaluation by your employer, an Authorized Operator’s Card is required per OSHA. Boom/scissor elevated work platforms and lifts Crane-suspended work platforms Cranes/mobile cranes Critical lifts Earth moving equipment Fall protection Fire extinguishers Forklifts and heavy equipment Scaffolding Steel erection/disassembly Trenching and excavation Utility tie-in Working on or near energized electrical equipment Confined Space Work Breathing Air/ Fresh Air Work Fire Watch/Hole Watch Specialized Operations 45Boom/Scissor Elevated Work Platforms: All lifts must be inspected by a qualified person and occur upon arrival at the job site and quarterly or every 200 hours, whichever comes first. A competent operator must inspect all controls and safety devices each day prior to use. Daily inspections must be documented. All lift operators shall be trained by a designated competent person. A copy of their field evaluation and training records will be kept on site in the field office. The contractor must also issue proof of qualification to each operator. Operators must have this proof of qualification with them at all times while operating that equipment. Fall Protection is REQUIRED 100% of the time Boom/Scissor Elevated Work Platforms 46Crane-suspended Work Platforms: Crane-suspended work platforms to hoist or suspend employees are acceptable in unique work situations. The Contractor must develop and implement a program for control and authorization of crane-suspended work platforms. The Contractor Site Manager or qualified designee must approve using a crane-suspended work platform using the Crane Suspended Work Platform Permit. Crane-suspended Work Platforms 47Cranes: All cranes shall be inspected upon arrival to the site by a qualified individual. Cranes shall be inspected daily prior to use. All cranes shall be equipped with: Annual permits Operator’s manual Fire extinguisher Operator logs Signal charts Load charts Anemometer required at the block or the jib Cranes 48Mobile Cranes: Only personnel who have been designated as a Qualified Operator, by Pandora, may operate mobile cranes. Operators must be assessed on site prior to operating equipment. Operator shall have a valid driver’s license. Weather conditions may limit crane use. A spotter is required at all times. A barricade is required on swing radius of all cranes. LMI and anti-two block device must be functional. All lifts require a JSEA. See the Pandora Site Safety Manual for additional requirements. Mobile Cranes 49Crane Lifts: Crane Lift Planning Must be Completed For All Lifts Lifts will be categorized based on level of risk Level of risk is based on review of load weight, percent of chart capacity, replacement time, replacement cost, and number of cranes used in the lift. Approvals required will vary based on risk level. Documentation required will also vary based on risk level. See the Pandora Site Safety Manual for additional requirements. Crane Lifts 50Fall Protection: Fall protection is required when working more than six feet above a floor or platform surface, or within six feet of any unguarded roof or platform edge more than six feet above ground level. “ This is called the six foot rule. ” Appropriate guarding and warning devices (chain, rope, cable) must be in place to protect workers from open holes and leading edges. Fall Protection Equipment MUST be inspected and logged Monthly and be clearly tagged as inspected. Each Individual MUST inspect his/her Fall Protection Equipment before each use. Fall Protection 51Fall Protection (cont.): Contracting companies shall train employees to: Recognize the hazards of falling from heights and avoid falls to lower levels through holes or openings in walking/working surfaces and walls. Training programs shall include prevention, control, and fall-arrest systems. Employee’s shall be trained to properly tie-off 100% of the time Training programs will include proper use of Tool Lanyards or other means of securing hand tool to prevent tools from falling. See the Pandora Site Safety Manual for additional requirements Fall Protection (cont.) 52Fire Extinguishers: The Contractor shall plan for and supply adequate fire extinguishers for all work areas, spaces, floors, and buildings where burning, welding, cutting or open-flame equipment is used or where smoking is allowed. Each welder, burner, or operator of open-flame equipment shall have an individual fire extinguisher within immediate reach of 10 feet (3 meters). Fire extinguishers for work on scaffolds and elevated platforms shall be readily available (within reach) to the workers. Fire extinguishers are also required for fueling operations. Each piece of motorized equipment shall be equipped with a fire extinguisher. Fire Extinguishers 53Fire Extinguishers (cont.): Fire extinguishers must be routinely inspected at least monthly by a competent person. Each extinguisher must have a durable tag securely attached, showing the date of the last thorough inspection, maintenance or recharge date and the initials or signature of the person who performs this service. Anyone using fire extinguishers must ensure the marking system remains legible between inspections. It is recommended that all lead persons, foremen, and supervisors be provided with “Hands On” training. All fire watch personnel shall be provided with training for the fire extinguishers they might be expected to use. Fire Extinguishers (cont.) 54Forklifts and Heavy Equipment: Only certified operators are allowed to operate equipment on this site. Each different "Type" of equipment will require individual certification for its operation. Each forklift attachment will require certification for its operation. The operator must carry his/her operator's card whenever operating a forklift or other equipment. All operators must have a valid driver’s license. Forklifts and Heavy Equipment 55Trenching and Excavation: The contractor shall develop a plan for control of excavations. Before beginning an excavation, the contractor must take the following minimal precautions: Complete an Excavation and Trenching Permit Remove or stabilize all surface encumbrances that create hazards to employees such as brush, spoil dirt, or boulders Erect either warning or protective barricades Keep spoil dirt and any material or equipment that may fall into an excavation at least three feet (1 meter) from the edge Remove loose rock or soil that could fall from the face of the excavation Protect, support, or remove underground installations or adjacent structures, as necessary, to protect employees and the environment Trenching and Excavation 56Working on or Near Energized Electrical Equipment: Four types of hazards Shock and burn due to contact or flashover Electrical flash burn from arc Blast injury resulting from electrical arcs Upset or shutdown of an operating area The electrically hazardous task must be analyzed considering two primary hazards - flash hazard and shock hazard. These hazards are identified by using boundaries based on energy levels. Performing work inside these boundaries requires training, planning, procedures, and PPE. See the Pandora Site Safety Manual for additional requirements Working on or Near Energized Electrical Equipment 57Tower Rescue: In the event of a fall incident, regardless of the medical condition of the employee, the supervisor or crew lead will be notified immediately. Prior to starting work, determine what information you will need to contact rescue and other emergency services. List this information on your JSEA. In the event an injured worker is unable to remove himself/herself from the tower, rescue procedures have been developed that will be put into action to ensure the employee receives an adequate and timely rescue. Rescue and Tower Climbing Procedures will have to be reviewed for compatibility with each manufactures tower configuration. The minimum requirement is to have at least two trained rescuers and one set of rescue equipment available within a 5 minute response time from the base of the tower. Have your designated rescuers listed on your JSEA. Tower Rescue 58Tower Rescue (cont.): Be prepared with additional information when you call for rescue or emergency medical service support. Examples of this need would be, but are not limited to: Site location Location of employee within the tower Condition of employee needing rescue / Incapacitated employee Ground assistance available Is there any additional rescue equipment needed? If first aid can be given before the injured employee is rescued from the tower, the rescuer should provide first aid before rescuing the employee. First aid should be administered to the injured employee by a trained employee until the local emergency medical team arrives. Tower Rescue (cont.) 59Hot Weather: Care must be taken to avoid any heat related illnesses such as heat stress, heat exhaustion or heat stroke. Make sure you are getting adequate rest before coming to work so that your body is ready for the work at hand. Acclimate new employees to working in extreme weather conditions. Stay hydrated throughout the day. Drink before you are thirsty. In extreme heat conditions, modify your work schedule as necessary to protect the employees. Hot Weather 60Cold Weather: When temperatures of extreme cold are expected, management will decide if work will take place. This will mostly come from the wind chill that will be created by high winds. Protect yourself from the cold, Hypothermia is a very constant and real danger on the job site. Cold Weather 61Contractor / Sub-Contractor Parking: All Employees will receive Parking instructions at the end of this Pandora Orientation. Bus Loading and Un-Loading shall be in a orderly fashion. Any Dis-Orderly Conduct may result in termination. Craft Personnel are Prohibited from parking in the Management Parking Lot. No Exceptions on; Parking, Bus Loading or Dis-Orderly Conduct on any Pandora location/Property. (subject to discipline and or Termination ) Contractor / Sub-Contractor Parking 62
