ETHICAL PROBLEM # 7 You are a psychiatric mental health nurse working in a Catholic hospital whose ethical and religious directives forbid abortion and aborting counseling. You are talking with a single woman recently hospitalized with bipolar disorder who is in first trimester of an unplanned pregnancy and who is expressing great ambivalence about continuing the pregnancy.You personally believe that your ethical obligation is to explore abortion as an option with this woman and to refer her to outside resources if she elects to abort. The charge nurse tells you that these are not appropriate options within this hospital.

ASSESSMENT :

DIAGNOSIS : 

DIAGNOSIS ►Moral Distress ► Ethical Principle: BENEFICENCE ► doing good for others and preventing harm

PLANNING : 

PLANNING Option # 1: The client will continue the unplanned pregnancy. Option # 2: The nurse will provide health teachings regarding the risk of pursuing abortion.

Ethical Principle: ►AUTONOMY- the right to self determination. ►NONMALEFICENCE- is an obligation not to “do no harm” ►BENEFICENCE- means “doing good” ►ACCOUNTABILITY AND RESPONSIBILITY- refers to the specific accountability or liability associated with the performance of duties of a particular role.

DECISION MAKING : 

DECISION MAKING The nurse will encourage the client to continue the unplanned pregnancy.

IMPLEMENTATION : 

IMPLEMENTATION ►Allow the client to verbalize her feelings. ►Assess what causes her ambivalence in continuing pregnancy. ►Encourage the client to continue the unplanned pregnancy. ► Discuss the advantages and disadvantages about abortion. ►Provide health teaching regarding pregnancy.

EVALUATON ► The preserving and protecting patient autonomy, dignity and rights. Maintaining a therapeutic and professional patient-nurse relationship The patient participating on resolving ethical issues.
