Specimen Preparation 6 steps to perform proper phlebotomy
SUPPLIES :Inventory performed a minimum of twice weekly (’Rotate’ stock...use oldest first!)
Check expiration dates on all phlebotomy materials (dispose of all expired inventory)
Ensure at least 1-2 week supply available SUPPLIES
REQUISITIONS :ALL information written/printed clearly
ALL information spelled correctly
ALL copies of supportive documents clearly legible (Med-Cal, Medi-Care, B-X...)
Double-Check all requisition info prior to submission REQUISITIONS
PATIENT PREP :Make patient as comfortable as possible
Ask patient if one arm is better than another?
Have all required tubes (and backups) at arm’s reach to avoid drawing problems
Make sure you have ammonia ampoule available PATIENT PREP
PHLEBOTOMY PROCEDURE #1 :Make sure patient is as comfortable as possible
Locate vein on arm...if tourniquet is required to locate, minimize time used
Apply/reapply tourniquet and swab arm with disinfectant...allow to dry or wipe off excess PHLEBOTOMY PROCEDURE #1
PHLEBOTOMY PROCEDURE #2 :Do NOT utilize ‘butterfly’ unless absolutely necessary (more chance of hemolysis)
Pumping/Squeezing of the hand/fist should not be too vigorous...slow & rhythmical
Remember to gently invert ALL tubes several times after drawing
Begin drawing blood utilizing the following ‘tube order:’ PHLEBOTOMY PROCEDURE #2
TOURNIQUET TIP :TOURNIQUET TIP ONLY leave tourniquet on long enough to begin the removal of the specimen(s). Prolonged use will falsely elevate certain analytes.
Unless absolutely necessary, remove tourniquet after blood begins to ‘free-flow.’ Should be no longer than 1.0 minute (if possible)
PATIENTAFTER-CARE :Ask the patient how they feel?
Apply bandaid over (or without) gauze
Inspect site for bleeding or hematoma
Help patient to their feet
Ask patient if they have any questions?
Thank the patient and escort them to the door PATIENTAFTER-CARE
