The German surgeon , Gustav Neuber (1852-1932) was the first design the OT attire & use APRON ,BOOTS & CAP during Surgery. An American Surgeon , William Halsted (1852-1970) the chief of surgery in john ,Hopkins university…used gloves during operation for the first time.Cont…......d: Cont…......d Johann von Mikulicz (1854-1905) A Romanian Surgeon invented special gloves for surgery. These gloves were made of cotton & were washed & dried by heat or sun before & b/w operations. By 1950 , Aseptic restrictions & strict OT dress code were adopted. All OT personnel were required to change street clothes & shoes before entering OT. In 1958 ,Disposable latex gloves were designed for the use during surgery.Definition : Definition OT attire is a part of aseptic environmental control in OT. It ensures protective barrier for the pt’s undergoing surgery as well as for the personnel during the surgical procedures.Purposes : Purposes The purpose of the OT attire is to provide protection from exposure to infection to the pt & also to the surgical team member. Transmission of micro-organism from hazardous material such as blood or body substances of the pt to the surgical team member. The infection such as cold ,cough ,skin infection from the surgical team member may also be transmitted to the pt.Component of OT attire : Component of OT attire1) Clean Attire : 1) Clean Attire It consist of the following: One piece half sleeve clean cotton gown for the female staff. 2 piece clean cotton pajamas & half sleeve ,collarless ,buttonless shirt for the male staff. A clean cotton head cover or cap A pair of clean OT sandals or shoes.Timing of use of clean Attire : Timing of use of clean Attire When a personnel arrives or enters OT he or she has to change into clean attire. The change of dress from street clothes & footwear's to OT attire & shoes is done in the changing room as it is situated at the entrance of the OT.Characteristics of Clean Attire : Characteristics of Clean Attire The OT dress is simple ,half sleeve ,collarless ,ankle length ,cotton gown for the female personnel. for a male staff ,the dress consist a clean pair of cotton pajamas waistline draw strings are to be tucked inside pajama to avoid their touching the sterile area. After changing the dress one has to put on the head cover & cap. it is made of cotton. It should be light weight & well fitted.Cont……….d: Cont ……….d It should cover the hair completely because hair is considered a to be gross contaminated. The head cover or the cap is to be put on before the scrub suit or sterile gown is done. So as to prevent contamination of the sterile gown by hair or dandruff. One has to change the street footwear. OT sandals should be without back strap & made up of soft leather ,must be well fitted & comfortable. After changing the street clothes to OT clean attire one can move in the semi restricted or restricted area in OT.The dress code for the OT Attire : The dress code for the OT Attire The people in street clothes after arrival in the OT should not be allowed to move beyond unrestricted zone in the OT. On arriving the OT ,personnel should change from the street clothes into clean OT gown or pajama suit ,clean head cover or cap ,OT shoes & sterile face mask. Each hospital should have an approved OT attire of specific texture ,color & design.2) Sterile attire: 2) Sterile attire Sterile attire consist of the followinga) Sterile face mask : a) Sterile face mask The scrub personnel have to wear sterile facemask which should cover both the nose & mouth .Cont……..d: Cont……..d The face mask is worn to minimize the airborne infection. a face mask has four long strings or tips at the four corners of the mask. The two strings of the head above the ears. Two lower corners strings run at the back of the head below the ear & are to be tied. Face mask are usually designed with pleats or a conical cup for a close fit.Cont………….d: Cont………….d The face mask is to be worn before entering the restricted area to filter droplets containing micro-organism expelled from the nasopharynx &the mouth of the surgical team members while breathing ,talking ,sneezing & coughing. A reusable cotton face mask is commonly used but they filter effectively as soon as they become damp. Disposable face mask are soft ,non irritating & comfortable. They do not obstruct respiration & are equally effective.# Code for the use of Face mask : # Code for the use of Face mask The sterile facemask should be handled just only by strings. The four corners strings of the face mask should be tied tightly at the back of the head. Two upper corners strings running over the ears & two lowers corners strings running below the ears ,just at the hairlines. One should avoid frequently handling & touching of the mask. Once the use of the mask is over ,it should not be lowered to hang loosely around the neck or should not be put into the pocket for reuse & should be discarded into specific container.Cont……….d: Cont……….d The face mask should be changed B/W the operations when they become soiled or damp. Talking & laughing should be kept to minimize while wearing a face mask.b) Sterile gown : b) Sterile gown After scrubbing the hands & drying the hands with a sterile towel ,the surgical team member have to wear the sterile scrub gown. It is a full sleeve ,full length collarless ,buttonless gown having a round neck ,front closed & a back opened with strings at the back which are to be tied by an assistant after the gown has been worn.c) Sterile pair of gloves : c) Sterile pair of gloves After the scrub gown is worn a sterile pair of gloves is to be worn & the cuffs of the gown sleeves are to be tucked inside the gloved cuffs. Each of these pair of attire has its specific role to combat infection & prevent contamination.Cont……….d: Cont……….d After wearing the sterile gown & gloves the surgical team members should restrict their stay in the restricted sterile zone of the OT & also should restrict their movement in OT.Cont……..d: Cont……..d The attire should be freshly laundered & cleaned. It should be washed dried & ironed preferably in the hospital laundry. The OT attire is to be worn with in the OT only. one should not go outside the OT in the attire. The OT attire is not to be carried Done because there is risk of contamination to household articles & to the family members.Other specific attires : Other specific attires They are used for specific purposes are as follows: A decontamination water or liquid proof apron : it is needed to be worn over the clean OT gown or pajamas suit. When there is risk of blood fluids of a pt splashing onto the gown of sterile team members.- Lead apron: - Lead apron may be worn below the sterile gown to protect against the radiation exposure when surgical team members are doing procedures such as implanting radioactive implants or removing such items.Cont………….d: Cont………….d Fire resistant gowns : may be worn for laser surgery. Eye wears or eye goggles : may be worn whenever a risks exist of blood fluids of the pt splashing into the eyes of the sterile members Sometimes is orthopedic operations ,bone cutting machines can splatter bone hips to the eyes. So it offers protection.
