Supplemental Life and AD&D Insurance Benefit Employee Spouse/DP Child Paid for by Supplemental Life 6 x annual pay, maximum $1M, $500,000 or 4 x annual pay guaranteed without Statement of Health (SOH) Lesser of $500k or employee basic + optional. $50k guaranteed without SOH $10,000 per child Employee Rates for coverage are based on covered persons age as of 12/31/2012 Flat rate, .45 per pay period Supplemental AD&D 10x annual pay in increments of $10,000 up to $1M Increments of $10,000 up to $500k $10,000 per child Employee
