Personal Health History:

Personal Health History For my first name my mom just like the name so she chose it for me. My middle name which is Gregory, was the name of my fathers best friend. My name is Irish and it means narrow body of water or peninsula. I was born in San Diego on April 8 th 1994

Personal Fitness and Nutrition (5 day scale) Studying per night: 6 hours Sleep per night: 45 hours TV watched per night: 5 hours Computer use per night: 10 hours Phone use per night: 15 hours Overall: 81 hours

Social and Emotional Health:

Social and Emotional Health I play tournament paintball and travel a lot for it so that’s all really do. I have two dogs. One is name Bear and is a German shepherd. The other is a named Lucy. Collie and My favorite trip was to Hawaii in 6 th grade with my brother and my mom. Schools: Teddy Bear tree house, Sundance Elementary, Mesa Verde, El Capitan High, Westview High.

Health Triangle:

Health Triangle Physical Play sports with friends and eat well. (probably not as well as I should be eating) Social Talk to friends. Do various things that are creative. Meet new people. Mental Stay positive. Always look at things with a good attitude.
