Originally WBI was told that the new pilot car training rule was in effect Jan 1, 2011 but there would be a 1 year phase in period.  We have been looking at the most cost effective way to do training.  We planned on doing all training in the final 3 months of 2011. :

Slide 14:

At a recent ATA safety meeting, we were told that DOT officers were actually going to start asking if pilot cars had received the training.  There is no official violation or citation. We are almost ready to start our pilot car training for our company.

What’s Wrong With This Picture?

Slide 24:

The labels on the boxes show incompatible materials: Spontaneously combustible materials and Corrosive materials.

Slide 26:

The hazmat items are missing the markings on the packages.

Slide 28:

The box on top of the lettuce states “Fragile Handle with Care.” It does not seem to be adequately secured to keep from shifting while in transit.

Slide 30:

The lettuce and the “Keep Frozen” box seem to be at odds with each other. In order to keep the top item frozen, the lettuce would result in spoilage, and if the lettuce is kept at its appropriate temperature, the frozen item would thaw.

Slide 31:

Following is a shortened example of a BBS observation

Slide 32:

Wrong Shipping Papers 10/11/11 When picking up a hazmat load from customer site, noticed the shipping papers had the wrong trailer number listed. The trailer also had 2 drums of acetone that weren’t on the BOL. Went back inside to get the correct shipping papers from the customer.

Accident / Incident / Autopsy:

Accident / Incident / Autopsy Incident: 9/30/11 While leaving customer site, driver attempted to move over to allow another vehicle more room to enter. When driver began to move forward, he caught his fender on a building, pulling the siding off.
