ACCIDENT INVESTIGATION PROCEDURES : 4 ACCIDENT INVESTIGATION PROCEDURES INCIDENT INVESTIGATION : 5 INCIDENT INVESTIGATION A detailed, systematic search to uncover the "who, what, when, where, why and how” of a loss producing event. PURPOSE OF INVESTIGATION : 6 PURPOSE OF INVESTIGATION To initiate changes or preventive measures to ensure the event is not repeated. DO NOT FIX BLAME : 7 DO NOT FIX BLAME The purpose of an accident/incident investigation is not to fix blame. It is conducted to obtain information. WHAT IS AN “ACCIDENT”? : 8 WHAT IS AN “ACCIDENT”? An unplanned and undesired event that results in personal injury or property damage. OSHA goes on to say that an illness that results from a single exposure is a "personal injury" but that illnesses resulting from long-time exposure do not fall into this category. OTHER DEFINITIONS : 9 OTHER DEFINITIONS Incidents - are unplanned and undesired events that adversely affect the completion of a task. Near misses - are incidents where no injury or property damage has occurred but where a slight difference in position or timing would have meant that damage or injury could have occurred. RESPONSIBILITIES : 10 RESPONSIBILITIES Supervisor: Thoroughly investigate all accidents/incidents/near misses. Maintain the scene to allow for proper investigation. Assist safety personnel in the investigation and analysis of accidents/incidents/near misses. RESPONSIBILITIES : 11 RESPONSIBILITIES Collateral Duty Safety Officer: Assist supervisors in investigations. Seek assistance from the Installation Safety Office as appropriate. IMMEDIATE ACTIONS : 12 IMMEDIATE ACTIONS Stabilize the accident scene. If possible, tend to the injured or try to protect the situation from becoming worse. However, do not jeopardize yourself or others. Notify the proper agency (Fire Dept., Safety Office, etc.). Secure the scene. Try to prevent unnecessary handling or moving of evidence. Notify the commander and installation safety office as soon as possible. Near-misses are often predictors of future serious accidents. Potential hazards can be corrected and employees can be offered training. ACCIDENT PYRAMID : 21 ACCIDENT PYRAMID Near Misses Injury - minor Injury - major Fatality NOTIFICATION : 22 NOTIFICATION (1) During normal duty hours. Telephonically notify the ISO, 295-7233, immediately of all accidents/incidents/near misses. (2) During non-duty hours/weekends/holidays. Notify the Staff Duty Officer (SDO) of a Class A or Class B accident or of a military fatality either on- or off-duty. The SDO will then notify the occupational safety and health manager IAW SDO instructions. REPORTING REQUIREMENTS : 23 REPORTING REQUIREMENTS 1. Submit FSH Form 96-E, on all accidents/incidents/near misses. 2. Report accidents >$2,000 damage on DA Form 285, U.S. Army Accident Investigation Report, and submit to the ISO within 7 days. 3. Fill out DA Form 285 for all military injuries and occupational illnesses > than one lost workday. Send to the ISO within 7 workdays. FSH FORM 96-E REPORT : 24 FSH FORM 96-E REPORT Three Sections: Section I - Data concerning affected person (May be omitted when reporting a near miss) Section II - Incident/Near Miss Section III - Supervisor’s comments and/or corrective actions ABREVIATED GROUND ACCIDENT REPORT (AGAR) : 25 ABREVIATED GROUND ACCIDENT REPORT (AGAR) Replaces DA Form 285 for : All OFF duty accidents C-D ON duty accidents A-B ON duty in combat CIVILIAN ACCIDENT REPORTING REQUIREMENTS : 26 CIVILIAN ACCIDENT REPORTING REQUIREMENTS 1.
