In promoting the optimal well being of our patients, we strive to create a safe atmosphere to support healing of the mind, body, and spirit, through compassionate care, innovative technology, and multidisciplinary professional collaboration. Mission Statement Nursing Care Delivery Model : Nursing Care Delivery Model Innovative combination of the community-based nursing center model and the collaborative patient care model. Purpose = Improve access for the vulnerable population at high risk for developing cardiac disease Complement the community-based model’s purpose to serve the poor and uninsured in a disease-specific group (Roussel, 2009) Managed by NPs and RNs serving as the care integrators of various health disciplines (Roussel, 2009) Congruency and efficient communication among various health care providers and their component systems are vital to the functioning of Healthy Healing Hearts Patient care coordinators (Roussel, 2009) Provide oversight of the interdisciplinary teams Foster communication Promote shared decision making Link quality, cost, and care delivery processes for effective allocation of resources Patient Care Coordinator Job Description : Patient Care Coordinator Job Description Position: The Patient Care Coordinator is a Registered Nurse who is competent in providing evidenced-based holistic care to patients with cardiac issues in a community clinic setting. Position Summary: The Registered Nurse coordinates patient care using the nursing process to provide safe, therapeutic, and efficient care while facilitating collaboration and communication of the interdisciplinary team. Reports to: Registered Nurse Team Knowledge Skills and Abilities Minimum Qualifications Responsibilities Interview Questions : Interview Questions Describe a time when you used your leadership skills to collaborate with other staff in resolving a difficult patient situation. How would you incorporate evidence-based research in your practice? How do you incorporate complementary or alternative therapy into the patient care?
