Plan Process improvement through information systems:

Emily Philip HAP 360-DL3 Plan Process improvement through information systems

Objective:

A nalyze the information that is needed to mange a depressed patient based on their genetic information . Identify the information needed on the first call to a clinic, first visit, between first and second visit in 8 weeks . Clearly identify the point in time when you recommend that the patient receive an antidepressant prescription Describe who will receive the information and how this information may lead to change in prescribed antidepressant . Summarize the optimal business process your information needs assessment suggests     Objective

Assumptions:

Patient is first seen by primary care physician at a private practice Patient suffers from depression Practice has HMO staff model Patient and providers have access to email and internet Patient is aware of his/her medical history Patient has active insurance Assumptions

First call to PCP:

Patient’s full name Patient’s date of birth Patients insurance information Reason for the appointment Scheduling the first appt. First call to PCP

First visit to PCP:

P atient will need to provide photo Id and insurance information Patients Demographics Current medication list Past medical History First visit to PCP

What happened in the first visit?:

In the first visit, after vitals are taken, the physician evaluated the patient by giving them a routine check up and evaluating the patients condition. The PCP then prescribes the patient an anti-depressant medication and sends it to the pharmacy. Once the pharmacy has received the prescription, the patient is able to pick it up. What happened in the first visit?

Follow up with PCP:

After a week the patient returns to see the primary care physician for a follow up appointment Patients Symptoms have not gotten any better from the antidepressant medication prescribed. At the follow up appointment, the primary care doctor refers the patient to see a psychiatrist Patient receives the referral and makes an appointment to see the psychiatrist Follow up with PCP

First visit to psychiatrist:

Valid ID and Insurance information Patients Demographics Referral Past medical history Medication list Patients symptoms First visit to psychiatrist

Continued::

Psychiatrist orders the patient to have a genetic screening Consent is given and patient goes to the lab to receive the generic screening Results will take at least 2-3 weeks While waiting for the results of the DNA screening, the psychiatrist tells to patient to continue taking the medication they are currently on Continued:

Second visit to the Psychiatrist:

Three weeks later when the genetic testing results are in: P atient schedules a second appointment to see the psychiatrist. The psychiatrist explains the results of the genetic testing to the patient The results reveal the kind, severity, and features of the patients depression The psychiatrist concludes that the patient should change the medication dosage and follow up with them in about 4 weeks Genetic results are updated in the EHR Second visit to the Psychiatrist

8 weeks from first visit :

The patient goes back to the psychiatrist to be evaluated by the psychiatrist on current medication dosage Patient shows signs that they are improving with the current medication dosage All results are entered into a EHR system 8 weeks from first visit

Subsequent visits:

Patient follows up with psychiatrist The current anti-depressant medication is still working for the patient Psychiatrist recommends the patient to stay on medication and to have talk therapy with a psychologist All information is entered into the EHR Treatment was based on the results of the genetic testing done by the lab Subsequent visits

Information exchange among multiple actors:

Information exchange among multiple actors

Documenting uSe Case:

Use case: Depression medication management Overview: The purpose of this use case is to allow the physician to add alternative treatment based on genetic information Primary Actor : Psychiatrist Secondary Actor: Sunrise lab (genetic laboratory) Starting point: The primary actor orders the genetic test and gathers the sample then sends it to the secondary actor who preforms the genetic testing and sends the results back to the primary actor. Ending point: The primary actor receives the report and decides the treatment. Patients EHR is updated. Information exchanges: The psychiatrist provides patients name, demographics, diagnosis, symptoms, history, and order for the genetic test to the the lab. The lab provides the genetic test report with patients genetic information and relays results to psychiatrist. Measurable results: Patient record is updated with genetic information and patients physicians are notified. Documenting uSe Case

Flow of Information:

Flow of Information Pharmacist: Receives prescriptions and updates medication information

summary:

T his process has shown what information is needed for an EHR How the information was accessed among multiple actors How genetic information can mange the treatment of a patient Collecting the patients information before the first visit makes the time utilized with the physician more efficient Using the EHR helps to send medical records to different actors who need it in order to provide optimal care to patient summary
