What happened in the first visit?:

In the first visit, after vitals are taken, the physician evaluated the patient by giving them a routine check up and evaluating the patients condition. The PCP then prescribes the patient an anti-depressant medication and sends it to the pharmacy. Once the pharmacy has received the prescription, the patient is able to pick it up. What happened in the first visit?

Follow up with PCP:

After a week the patient returns to see the primary care physician for a follow up appointment Patients Symptoms have not gotten any better from the antidepressant medication prescribed. At the follow up appointment, the primary care doctor refers the patient to see a psychiatrist Patient receives the referral and makes an appointment to see the psychiatrist Follow up with PCP

Psychiatrist orders the patient to have a genetic screening Consent is given and patient goes to the lab to receive the generic screening Results will take at least 2-3 weeks While waiting for the results of the DNA screening, the psychiatrist tells to patient to continue taking the medication they are currently on Continued:

Second visit to the Psychiatrist:

Three weeks later when the genetic testing results are in: P atient schedules a second appointment to see the psychiatrist. The psychiatrist explains the results of the genetic testing to the patient The results reveal the kind, severity, and features of the patients depression The psychiatrist concludes that the patient should change the medication dosage and follow up with them in about 4 weeks Genetic results are updated in the EHR Second visit to the Psychiatrist

8 weeks from first visit :

The patient goes back to the psychiatrist to be evaluated by the psychiatrist on current medication dosage Patient shows signs that they are improving with the current medication dosage All results are entered into a EHR system 8 weeks from first visit

Subsequent visits:

Patient follows up with psychiatrist The current anti-depressant medication is still working for the patient Psychiatrist recommends the patient to stay on medication and to have talk therapy with a psychologist All information is entered into the EHR Treatment was based on the results of the genetic testing done by the lab Subsequent visits

Use case: Depression medication management Overview: The purpose of this use case is to allow the physician to add alternative treatment based on genetic information Primary Actor : Psychiatrist Secondary Actor: Sunrise lab (genetic laboratory) Starting point: The primary actor orders the genetic test and gathers the sample then sends it to the secondary actor who preforms the genetic testing and sends the results back to the primary actor. Ending point: The primary actor receives the report and decides the treatment. Patients EHR is updated. Information exchanges: The psychiatrist provides patients name, demographics, diagnosis, symptoms, history, and order for the genetic test to the the lab. The lab provides the genetic test report with patients genetic information and relays results to psychiatrist.
