NKS Medical rehabilitation tend to be physical in sense and medically oriented Social Rehabilitation focus on social health contains social, psychological, economic health integrated to society and doing the previous social activities. Family is the major player in social rehabilitation.

NKS The principles of good medical and social care for older people are: Involvement of older people Promote good health in later life and prevention of illness Reduction of disability Support older people in their own homes, preservation of dignity, autonomy and respect: Old age itself is not an adequate explanation or cause of physical or mental illness

NKS MULTI-DISCIPLINARY (MDT) Elderly should have access to the skills and expertise of Geriatrician and the specialized services of a Multi- Disciplinary Team (Royal College of Physician London 1994). Every older persons receiving intermediate care needs review by a multi- disciplinary team including a Doctor who specializes in the care of older people. CORE Members: Consultant Geriatrician - Leader Nurses Physiotherapist Occupational Therapist Social Workers Patients /Carer Associate Members: Speech Therapist Psychologist Nutritionist Psychiatrist Pathologies Priest Pharmacist

NKS IMPORTANCE OF MDT MDT is widely practiced Prevents bed blockers and expedites the treatment and rehabilitation Provides shared goals of different profession and facilitates exchanges of information & provides holistic and quality of treatment. Proven positive results in terms of diagnosis/prognosis of patients and the necessary intervention

NKS CHALLENGES OF MDT Three barriers that hindered teamwork: differing perceptions of teamwork; different levels of skills acquisitions to function as a team member; and dominance of medical power that influenced interaction in teams.

NKS GERIATRICIAN Medical Doctor who specialized in older person Supervises the aspect of treatment and social and medical rehabilitation of the patients. Act as the facilitator and solicits the intervention and progress of rehabilitation and means of achieving the goals of the MDT. Provide direction to the team taking into considerations the suggestions of the MDT members.

NKS PHYSIOTHERAPIST Enabling older people to use a number of body systems to enhance mobility and independent function. Contribute by helping older people to remain comfortable and pain free. Provides services for elderly people with disability problems which limit mobility and function to restore maximum independence.

NKS OCCUPATIONAL THERAPHY Provides assessments, Individual Treatment Session, Group session Conduct social/leisure/recreational activities. Facilitate Community Involvement, Divisional Activities, Family Training Sessions and it focus on restoring the daily activities of older persons (bathing etc.) Identify vulnerable areas in the residence that are not elderly friendly to prevent falls.

NKS GERIATRIC SOCIAL WORKERS/PSYCHOLOGIST Focus on Social Rehabilitation Counseling to the older person about the sudden change and effects of the degenerative sickness. Provide information and education to the family Provide networks/support system Promote/organize community participation

NKS GERIATRIC NURSES Provide updates on the MDT about the progress of treatment and rehabilitation of the older person. Inform the MDT about the needed intervention and problems that needs immediate medical or social intervention. Geriatric Nurse must have specialized education on geriatrics and gerontology.

NKS Emphasis should be on the MDT nature of education and training. Government have the primary responsibility for setting and monitoring standards of health care as well as providing health care for older persons. Promotion of Maximum Independence- Rehabilitation of older person. To achieve this, we need to implement multi-disciplinary approach in treating the elderly patients and to recognize that every members of the MDT has distinct roles and contributions in the achievements of the shared goals of the MDT Conclusions

NKS MDT not only in tertiary/secondary hospitals but also in the community Urgent need to expand educational opportunities in the field of Geriatrics and Gerontology both in medical and social service sectors. Informal caregivers also need access to information and basic training on the care of older persons. Regulate care-giving schools that they must pursue quality of education with the guidance of the MDT and expertise of Geriatrician. [1] WHO definition of Palliative care, active total care of patients whose disease is not responsive to curative treatment namely by controlling pain and other symptoms of the disease and offering psychological, social and spiritual support to patients and their families.
