Preparing for your test: 

Preparing for your test Read all directions on the direction sheet Three days before collection, stop all antifungal or antibiotic medication Plan to collect on Sunday through Thursday to allow for mailing Find space in your refrigerator for test kit storage, if necessary

What is in your kit?: 

What is in your kit? 2 direction sheets ( Metametrix and C4OH…use C4OH) 1 plastic biohazard bag containing 2 disposable gloves 1 “ french fry” collection tray 3 collection vials 1 absorbent pad 1 b lack waste disposal bag 1 requisition form Fed Ex shipping items 1 mailing bag 1 adhesive shipping label 1 US shipping instructions

Ready to begin?: 

Ready to begin? Have you read and understood all of the instructions? Do you have all necessary test kit supplies? Have you finished antifungal and antibiotic medications three days prior? YES! You are now ready to being your collection.

Collecting the sample: 

Collecting the sample Put on disposable gloves Collect a sample of your stool into the container For each of the three vials, remove the cap and use the attached spoon to transfer sample into the vial Take multiple portions from different areas DO NOT OVERFILL the vials

Preparing the samples: 

Preparing the samples Screw on each cap tightly Shake the vials vigorously for 30 seconds On each vial you must write, in permanent marker: Your name Date of collection Stool consistency (see chart on directions) Place the vials into the plastic biohazard bag with the absorbent pad and seal

Cleaning up: 

Cleaning up Dispose of remaining specimen (either in the toilet or black waste bag) Dispose of the gloves and collection tray in the black bag, seal, and place bag in the garbage

Filling out the requisition form: 

Filling out the requisition form Record stool consistency and date of collection in section 4 Fill out your demographic information (highlighted portions only) in section 3 Do not worry about the back of the form Medicare patients: sign the bottom of the page below section 7

Preparing for shipping: 

Preparing for shipping Place biohazard bag containing your samples into the cardboard box Did you write your name and the stool consistency on all three vials? Place requisition form into cardboard box Did you fill out the appropriate sections? Everything else can be taken out of box Close cardboard box and place in FedEx bag Affix shipping label to FedEx shipping bag

Scheduling FedEx pickup: 

Scheduling FedEx pickup Call FedEx as per instructions in your test kit You MAY NOT use a FedEx drop box For weekend collection, you must hold onto the specimen until Monday for pickup Ship specimen ASAP, preferably on the same day as collection Keep specimen in the refrigerator until pickup
