RAT 2011 Flowchart RETRIEVAL OF RESULTS Division Testing Coordinator (ESI) District Supervisor ( PSDS ) Cluster Head Chief Examiner (Head of the School Examined) School QAA Team (Led by the School Head of the Room Examiners) Room Examiner (Teachers from other schools) Pupils/Students Takes charge of checking the answer sheets of pupils/students Prepares Form XIV and % of Correct Responses in 3 copies Submits the accomplished Form XIV to the School QAA for review Reviews the computation of Forms XIV and % of Correct Responses Submits reviewed Forms XIV in 3 copies to the Chief Examiner (1 copy for the class adviser, 1 copy for CE, and 1 copy for PSDS/CH) Prepares Form XV and School Consolidation of % of Correct Responses Submits them to the District QAA for district review Prepares Form XVI and District /Cluster Consolidation of % of Correct Responses Submits Results to the Division/Cluster QAA Team for division review Reviews the computation of Forms XVI Submits reviewed Forms XVI to the Division Testing Coordinator RFTAT Prepares Regional Consolidation of RAT Results Submits the Results to the Regional Director Presents/Furnishes results to all Schools Divisions and DepEd Central Office District/Cluster QAA Team (Led by the PSDS/Cluster Head) Division QAA Team (Headed by DTC) Reviews the computation of Forms XV (School Consolidation of RAT Results) Submits reviewed Forms XV to the District Supervisor QAAD Prepares Division Consolidation of RAT Results and Division Consolidation of % of Correct Responses Refers DTC Consolidation to the Division QAA Team for checking Submits Division Consolidation to RFTAT Reviews the Division Consolidation of RAT Results and Division Consolidation of % of Correct Responses and submits them to QAA
