the majority of anesthesia services should be billed using codes in the range of 00100–01999. i f you bill for the administration of anesthesia for multiple surgical procedures performed during the same operative session, submit only one anesthesia code. Choose the anesthesia code representing the most complicated procedure. The time reported is the combined total for all the procedures.



Anesthesia Modifiers :

Anesthesia Modifiers See page 34 prior to Anesthesia Chapter Modifier Description Base P1 Normal Healthy patient 0 P2 Patient with mild systemic disease 0 P3 Patient with severe systemic disease 1 P4 Patient with severe systemic disease that is a constant threat to life 2 P5 A moribund patient who is not expected to survive without the operation 3 P6 Declared brain dead patient whose organs are being removed for donor purposes. 0

Anesthesia Modifiers:

Anesthesia Modifiers Beginning with date of service February 1, 2012, anesthesia services 00100-01999* will be required to have one of the following anesthesia modifiers. AA (Anesthesia services performed personally by an anesthesiologist) AD (Medical supervision by a physician: more than 4 concurrent anesthesia procedures) G8 (Monitored anesthesia care for deep, complex, complicated, or markedly invasive surgical procedure) G9 (Monitored anesthesia care for patient who has history of severe cardio-pulmonary condition) QK (Medical direction of 2, 3, or 4 concurrent anesthesia procedures involving qualified individuals) QS (Monitored anesthesia care service) QX (CRNA Service: with medical direction by a physician) QY (Medical direction of one CRNA by an anesthesiologist) QZ (CRNA Service: without medical direction by a physician)

Time Reporting:

Time Reporting Begins when the anesthesiologist begins to prepare the patient for the induction of anesthesia n the operating room (or equivalent area) and ends when the anesthesiologist is no longer in personal attendance. When Multiple procedures are performed during a single anesthetic administration, the code representing the most complex procedure is reported. The time is the comined total for all procedures.

Most Extensive Procedures :

Most Extensive Procedures When procedures are performed together that are basically the same or performed on the same site but are qualified by an increased level of complexity, the less extensive procedure is bundled into the more extensive procedure.

Categories Part 1 Head Neck Thorax – lies between the head and the abdomen ( Chest, Shoulder) Intrathoracic - Situated or occurring within the thorax or chest. Mostly Heart and Lung Spine and Spinal Cord Upper Abdomen – gastrointenstinal , hernia in upper ab , abdominal wall, liver, lap band

Categories Part 2 Lower Abdomen – Hernia in Lower Ab , urinary tract, kideny , lower ab wall Perineum - surface region in both males and females between the pubic symphysis and the coccyx . Involves prostate, penile, vaginal procedures Pelvis – Involves pelvic bone and joint

Categories Part 3 Leg has three categories Upper leg Knee Lower leg (below knee, includes ankle and foot) Arm has 4 categories Shoulder Upper arm and Elbow Forearm, Wrist and Hand

Categories Part 4:

Categories Part 4 Radiological Procedures Burn Excision (removal) or Debridement (removal of dead or damaged skin) Obstetric – Child birth
