Assessment:
Assessment History- anger issues, disrespectful to grandmother, waking up at night. Strenghts - makes good grades at school and behaves well at school. Stated that he wants help with his behaviors. Needs- anger management skills, positive role models in his life. PIE- child was removed by DSS because mother was using drugs in front of the children. Has a lot of resentment towards his mother. Theory used- Cognitive Behavior Theory
Intervention:
Intervention Assigned to an Intensive in home care team to address anger management skills. The AP, QP, and LP will meet with the client and his family to address these issues. Developed a person centered plan to address the issues and goals of the client and client system.
