Save this application form by right click the form Print the form from the printer and fill out the form with necessary particulars This section is your personal info Personal referral info. Scan the form and email to amt.trader.support@gmail.com; please stated the e/mail subject title as “New Student Application for ROSLI BIN KASSIM (your name)”. Just an example name If you are not convenient to use email or don’t know how to use e/mail to send the form, you can always handed your form to your personal referral (the person who introduce you to this business, usually your up liner) to hand in to our management team.

The Payment Please hand in your student fees RM 700 or USD 200 to proceed with the application. Payment Method Method 1: write cheque to “AMT MATRIX TRADING” Method 2: bank in money to “ AMT MATRIX TRADING” bank institution: RHB bank with acc no.21232100047917 Method 3: handed cash to your personal referral (this method will apply only when your personal referral has a sign up pin ready and be able to access to internet on the spot.)

Log on to www.fxcm.co.uk to register and open a live account.

Click to choose your residential country and then click “ ” icon for METATRADER 4.

Click “USD Account” Then “Individual Account” And then “Open an account”

Please fill in Commission Acknowledge Form CA and stated YEW KIM CHOON as your agent Commission USD $3.00 (because all our account is mini account in FXCM) It will work out to be the same. USD $3.00 Fill in necessary information

Please scan the “Commission Acknowledge Form CA” that you just filled in and email to admin@fxcm.co.uk You also have to send the same scanned copy of “Commission Acknowledge Form CA” to amt.trader.support@gmail.com (With this e/mail you need to carefully state down Your Name, Your FXCM trading account number and password In order for our trader team to keep track of your account please send your name, your AMT user id, your FXCM trading account number and password to amtrader001@gmail.com
