Coach Volunteer Forms: Coach Volunteer Forms All coaches must provide two signed copies of the volunteer registration form
Access using WebYS:
click “Coach”
click “Volunteer Form”
complete, print two copies, sign and mail to:

Coach is responsible for getting signed forms from parents (due to online registration/payment)

Drops, Adds, Parent Requests: Drops, Adds, Parent Requests Inform DC of:
player dropping out (confirm with parent!)
parent requesting team change
e.g., practice day conflict
player wanting to join your team

DC handles all team assignments/changes
do not allow unassigned players to practice
