4.12 HOLD – A Hold is a condition, wherein a specific circuit or equipment is isolated from all known sources of energy by having all switches or valves necessary for such isolation placed in the “Protective Position” and tagged. A Hold is not “ON,” (employee protection is not in place), and work shall not proceed until the Hold has been granted by a Control Authority to an Authorized Employee. The Hold procedure is designed exclusively for personal protection and will not be used for any other purpose.

Compliance with the lockout/tagout standard (29 CFR 1910.147) prevents an estimated 120 fatalities and 50,000 injuries each year. Workers injured on the job from exposure to hazardous energy lose an average of 24 workdays for recuperation. In a study conducted by the United Auto Workers (UAW), 20% of the fatalities (83 of 414) that occurred among their members between 1973 and 1995 were attributed to inadequate hazardous energy control procedures specifically, lockout/tagout procedures. KCP&L Supply DivisionHold Procedure 4.11 GRANTED HOLD – The condition in which protection has been applied, equipment has been deenergized and an Authorized Employee has been notified by the Control Authority that the Hold is on. Once granted, it continues to exist until the Authorized Employee properly surrenders the Hold to the Control Authority. Any number of Authorized Employees may be granted a Hold on the same circuit or equipment.

KCP&L Supply DivisionHold Procedure OSHA Standard 1910.147(a)(1)(i) and 1910.269 (d) These standards cover the servicing and maintenance of machines and equipment in which the unexpected energization or start up or release of stored energy could cause injury to employees. These standards establish minimum performance requirements for the control of hazardous energy. KCPL Safety Rule SR-004 7.1.3 It is the responsibility of every employee of all operating divisions of the Kansas City Power & Light Company to familiarize themselves with and understand the Hold procedure and to fully comply with the various provisions outlined herein.

KCP&L Supply DivisionHold Procedure 7.1.1 All circuits and equipment, including electrical and mechanical apparatus, steam lines, etc., are classified as either ENERGIZED or DEENERGIZED. 7.1.2 All circuits and equipment must be considered ENERGIZED at all times unless fully protected in accordance with the Hold procedure. 7.1.4 A Hold cannot be granted to anyone other than an AUTHORIZED EMPLOYEE of KCP&L Company.

Definitions AUTHORIZED EMPLOYEE An Authorized Employee is any KCP&L employee who, by virtue of current qualification, has authority to secure a Hold on a specific circuit or equipment that is to be worked as deenergized. CONTROL AUTHORITY The Control Authority is the person responsible for approving Holds, for sequence of switching and tagging operations and for granting Holds on circuits and equipment.

Definitions SWITCHMAN A trained Operator respon- sible for the isolation, tagging and reenergizing of equipment as prescribed on the Limits & Sequence sections of the Hold. APPOINTED KCP&L REPRESENTATIVE An Appointed KCP&L employee who is designated to direct contractors. This can be a Construction Services Administrator, Operations or Mainten- ance Supervisor, Engineer, Planner, Step-up Supervisor or any other designated KCP&L employee.

Definitions AFFECTED EMPLOYEE An employee whose job requires him or her to operate or use a machine or equipment on which servicing or maintenance is being performed under the Hold procedure or whose job requires him or her to work in an area in which such servicing or maintenance is being performed.

Definitions RED TAG An approved red tag which can be securely fastened with a one-time use, uniquely identified, self-locking plastic tie to an isolating device placed in the protective position. Tags placed in harsh environments shall be protected. LOCKING DEVICE A one-time use, uniquely identified, self-locking plastic tie or combination tie and non-conductive device, (i.e. hasp etc.), that utilizes a positive means to hold an energy isolating device in the safe position and prevent the energizing of a machine or equipment.

Each Hold is identified by a unique number The “Submitted By” line allows an Authorized Employee to electronically submit a Hold and have isolation completed before signing the requested by line. The “Planned By” and “For” allows an approved Planner or Supervisor to request a Hold with switching to be completed before Authorized Employee(s) sign on the Hold. These lines Must contain the names of the employee(s) planning the Hold and the employee(s) who may complete the work.

After Switching has been comp-leted, the Control Authority shall contact an Authorized Employee on the Hold, communicate known abnormal conditions and potential hazards and that the Hold has been placed. 7.7.1. Only after completion of these steps, will the Hold be “granted”. After granting, the Hold Form becomes a legal document.

Beright 9834 FIELD CHECKING ISOLATION RELEASE After collecting all tags and locking devices, the Switchman will return with them to the Control room. 456437 Rackout & tag 9A BFP Mtr Brkr Power Center 2Z, Cubicle 49 J. Operator (Cntrl Auth.) The Control Authority will ensure that the numbers on the tags, the locking devices and the Hold form all match. Then he or she will sign and initial the tag. At that time , the locking devices may be disposed of. Tags and all permits must be stapled to the original Hold form and sent to the station Safety Coordinator where they will be audited and kept for 5 years.

Special Instructions : 

Special Instructions 7.10. Surrendering the Hold when Authorized Employee(s) are not on site 7.10.1 When an Authorized Employee is not at the facility and is unable to properly surrender a Hold, the immediate Supervisor on shift, (may be Mechanical or Operational), shall make a visual inspection of equipment, strictly comply with all articles of paragraph 7.9, and take full responsibility for the surrender of the Hold. This responsibility includes submitting a completed and signed KCP&L Hold Tag Removal form to the Control Authority. 7.10.2 After thorough review, the Control Authority will assign a Switch Person to remove the tag. 7.10.3 Copies of the completed KCP&L Hold Tag Removal form, a copy of the Shift Foreman’s log entry and an e-mail alert shall be sent during the same shift to the Plant Manager and Safety Coordinator for review. 7.10.4 The original copy shall be attached to the Hold. 7.10.5 The Authorized Employee(s) shall be notified before resuming work that his or her Hold has been released. However, this does not eliminate the responsibility of the Authorized Employee(s) to verify the status of this Hold before continuing work.

Special Instructions 7.3.3 When errors are made during the request stage the Hold, above the double line, the Hold form MUST be disposed of by the requesting employee and a new form used. 7.8.4. Should it be discovered that the limits of a Hold provide inadequate worker protection; all work being conducted under the Hold shall be suspended. 7.8.4.1. The Control Authority shall be notified and a request shall be made for additional limits. 7.8.4.2. The Control Authority shall add the additional limit(s) and notify each Authorized Employee whose signature is on the Requested By line that the Hold will be modified. 7.8.4.3. The Control Authority shall not delete limit(s) from the original request. 7.8.4.4. By order of the Control Authority, the Switch Person will isolate the additional point(s) and all Authorized Employees shall field check the additional isolation points.

Special Instructions Additional Limit(s) WILL not be added to a Hold if the additional limit(s) change the scope of work.

Special Instructions When a Hold is planned for the next day’s work, the Hold can be approv- ed and isolations can be performed the day before. An Authorized Employee must sign on the first request line and the Hold must be approved by the Control Authority. The Control Authority will sign and date the Hold once Granting has been completed.

Special Instructions Each time an Authorized Employee signs the request line a field check must be completed and tags initialed.

Special Instructions When tags are lost, missing or unrecognizable, a duplicate tag will be made and marked as a duplicate. Explanations will be made in the comment section of the Hold explaining the duplication. 9.4.2 Should an employee find an unattached Hold tag in the field, i.e. on the floor or on the ground, the employee shall immediately contact or send word to the Control Room and “stand firm” to warn others in the area that their protection from energized components may be compromised. If the employee is unable to communicate directly from the area, they should travel the least distance possible to relay the information and return to the area until Operations takes control.

Special Instructions When a Confined Space Permit or Shorts and grounds Tags (indicating shorts and grounds have been applied) are attached to the Hold, the Hold SHALL not be released until these conditions are cleared.
