Step 1: MySLU -> Banner Self Service :
Login to your MySLU account
Click on the Tools tab
Click on Banner Self Service
Or log directly into Banner Self Service Step 1: MySLU -> Banner Self Service 2 Click to Proceed
Step 2: Access Transcript Request Form :
Click on Student
Click on Student Records
Click on Request A Transcript 3 Step 2: Access Transcript Request Form Click to Proceed
Step 3: Select a Recipient :
Select one of your addresses from Option 1
OR
Enter the Recipient’s Name in Option 2
Click Continue 4 Step 3: Select a Recipient Click to Proceed
Step 4: Finish Entering Address :
If you selected Option 1 this is a verification
If you selected Option 2
Finish entering the address information
Click Continue when complete 5 Step 4: Finish Entering Address Click to Proceed
Step 5: Quantity and Submission :
Enter the Number of Copies desired
No more than 5
Click Continue
Review and click Submit Request 6 Step 5: Quantity and Submission Click to Proceed
You have Completed the Request :
You have now completed your transcript request
You may view the print date of all transcript requests by accessing “Transcript Request Status” located on the Student Records Menu 7 You have Completed the Request Click to Proceed
