Present Scenario Patients commonly visit Hospital and GP without previous medical record Incomplete or very old or illegible handwriting Colour of the tablets (red or white) Each hospital admission creates new Pt record of same Pt. In the middle of an emergency, there’s no room for errors

Electronic Health RecordHow important?: 

Electronic Health Record How important? Do you want me to remember my medical history? Do you want me to remember all my current medication? SOLUTION 

PeHD: 

PeHD It is a portable, decentralized, patient centric, medical record which provides an organised summary of personal medical information for presentation to health care providers It is multifunctional and multipurpose, includes text, images, sounds, and other multimedia content. It not only supports doctors to have access to the patients’ medical records, but also for Hospital treatment purposes, Government bodies for statistical purposes, and research purposes.

PeHD: 

PeHD 1 Gb memory portable device + same copy of data stored in server. Software with different interfaces for input and displaying data Public Data: Chronic disease, medication, contact number Private Data: medical information, personal notes (complaints to Doctor) Data base of medical record with link to server URL andamp; Synchron. facility Specifications

Benefits for Patients :: 

Consumers can recheck medication schedules for confirming review date, investigation date and vaccination date. They can access their medication list and purchase same drug and dose. They can receive lab results directly to their PeHD via server. Benefits for Patients :

Benefits for Health Carers (Doctors / Nurses / Lab Tech...):: 

Doctors can access the PeHD which will avoid repeating investigation done before, saving time for diagnosing and work load for lab technicians. Contact electronically for second opinion. Nurses can access the PeHD if the patient is admitted in the hospital and review patient history regarding previous treatment and list of medications.
