Must have US Citizenship or permanent resident status (green card)
Valid US driver’s license
No current suspensions
No DUIs in last 3 years
Unacceptable health conditions/medical history - treatment or diagnosis in past 10 years (any that would require an APS or Blood/Urine samples)
Diabetes, cancer, heart attack, heart bypass/angioplasty, sleep apnea, stroke/TIA
Acceptable health conditions/medical history
Anxiety/depression - single med, no disability
Asthma - mild, no steroid use, no ER or hospital visits and no time off work
Infrequent attacks, 1-5 per year that are controlled with inhalers or simple bronchodilator medication (ie., episodes brought on by allergies, cold, exercise.
Hypertension/high blood pressure - controlled
