Sheppard AFB Investigation SFTH POL Tech Team Dispatched 16 Jul 98 TK #33 (issue) tested at >2000 PS/M at the top and middle and 1700 PS/M at the bottom TK #33 immediately placed on QC hold TK #34 tested at top, middle, & bottom and the results were 494, 493, 490 PS/M respectively TK #34 on spec and designated as the issue tank Tank #32 Defueled +100 will clean rust & dirt from tank Rust & dirt may contaminate fuel Inactive status exacerbates problem

Sheppard AFB Results: 

Sheppard AFB Results Tank #34 now receipt & issue tank DER-A requested to supply fuel w/o SDA Hess now the sole source supplier Delivering 30 trucks a day fuel w/o SDA Tank #33 was >2000 pS/m after receiving 182K fuel w/o SDA (492K gls high CU) Tank #34 now down to 90 pS/m Tank #32 reissued to A/C thru Pall F/S Tank #33 transfer to Tk #32 for dilution

Summary JP-8+100 was not the culprit Sheppard only +100 AETC base with high conductivity problem Only Tank #33 contains high conductivity After dilution (.58:1) SDA still >2000 pS/m Tank #33 ullage probably not adequate to bring back on specification Tank #32 delivered to A/C T-37’s 400 Hz Honeywell not affected (T-38’s 400 Hz Simmons Precision) Fly without +100 and SDA until all fuel is back on specificaton

Sheppard AFB Recommendations Trend Western to increase quality surveillance on receipt Request DER-A to limit maximum level of conductivity injection to 450 pS/m Request DER-A to investigate source of high SDA and apply corrective measures DESC/SERVICES/JCS AF reevaluate Inventory Policies

Charleston AFB SC Apr 98 - high PC, Four receipt filter/separator element change-outs Migrated to the flight Line Six hydrant hose trucks shutdown at the aircraft Aug 98 - high PC, AMC directed to receive high PC fuel (12 mg/gal) To be cleaned up by filters

Charleston AFB SC 30 Sep 98 - high water, pc and 25 min filtration, Failed visual solids upstream of F/S @ 1 hour after pipeline start Passed all tests downstream of F/S @ 1 hour after pipeline start Six hours after pipeline start visual water dP exceeded limit system shut down to replace elements New elements saturated with water only after 6 hours
