A1, Introduction
Provides a definition of when this instruction is applicable - while underway and pierside. Other requirements (i.e., OPNAVINST 5100.23G) may have precedence during availabilities (e.g., energy control, gas-free, hazmat, etc.)
Adds references and includes traffic, RODS, explosive and aviation safety as elements of SOH.
Clarifies precedence of standards and allows for use of nationally recognized consensus standard.
A2, SOH Program Organization and Responsibilities
Requires COMUSFLTFORCOM, COMPACFLT, COMSC, and COMSPECWARCOM to maintain a qualified staff of SOH professionals.
Requires ISICs to ensure oversight inspections are conducted every 3 years including review of traffic safety, recreational/off-duty safety and ORM. INSURV/Safety Center Surveys meet this requirement provided the ship’s CO provides a copy of the report and corrective actions within 30 days to the ISIC.
Requires prompt investigation of mishaps and designation of WESS Safety Authorities.

IH Survey Strategy
To assess potential health risks faced by Navy personnel by understanding their exposures, to differentiate between acceptable and unacceptable exposures, and to control unacceptable exposures.
To establish and document a historical record of exposure levels for Navy personnel and to communicate exposure monitoring results.
To ensure and demonstrate compliance with safety and health exposure criteria.
To provide a basis for medical surveillance examinations.

Baseline IH Survey
A detailed hazard evaluation of all operations (hazardous chemicals and/or harmful ergonomic, physical (e.g., noise, heat stress) or biological agents).
The survey shall include a list of exposures that require routine monitoring (e.g., exposure monitoring plan).
An assessment of the effectiveness of ventilation systems.
Recommendations for PPE, respiratory protection, emergency equipment (eyewash stations and deluge showers), posting of hazards, and actions on specific survey results (noise, air sampling, etc.), including:
A list of noise hazardous areas/equipment.
A list of eye hazardous areas/processes and emergency eyewash and deluge shower locations.
A list of each area/process requiring respiratory protection.
Medical surveillance requirements.

Periodic IH Survey
Required every 2 years.
New or modified equipment or processes.
Introduction of new hazardous chemicals and/or harmful physical or biological agents.
Deterioration of existing controls (e.g., ventilation) which degrade over time.
Post shipyard availability period.
For submarines, detailed atmosphere sampling, per ACM, is required at the completion of a major shipyard availability or prior to post scheduled availability (PSA) to demonstrate that there are no built in contaminants as a result of construction activities and proper functioning of the atmosphere control system.

Recommends assigning a safety observer.
Identifies the safety harness as a full-body harness vice a parachute type safety harness.
For working aloft, requires wearing a full-body harness with safety lanyard and climber safety device when climber safety rail is installed. Requires wearing a full-body harness and double safety lanyard configuration if no climber safety rail is installed.
Adds Section C0807 to discuss Procedures for Working in Vertical Trunks.
Appendices C8-A and C8-B - Updates checklists for working aloft and over-the-side to incorporate requiring an operational risk management assessment and brief prior to every evolution. Adds Appendix C8-C, a checklist for working in a trunk.

Specifies electrical safety gloves vice rubber gloves and requires PMS prior to use.

Updates requirements for maintenance of lead-acid batteries including PPE requirements and storage and disposal of battery acid in accordance with guidance.
Adds Section C0909 on Electrically and Electronically Safe Work Benches. Requires work benches to be grounded and have grounding leads. Requires a quick disconnect device to be installed. Requires labeling as “electrically safe”. Requires signs on the rescue of personnel and CPR to be posted adjacent.
Adds Section C0910 on Tag-Out.
